Yaşam boyu yaygınlığı %9-20 olarak bildirilen major depresif bozukluk depreşme, yineleme ve yüksek süreğenleşme ile seyreden, fiziksel, ruhsal, sosyal ve mesleki yeti yitimine neden olan bir hastalıktır. Tedavide ilaç etkinliği ile ilgili yapılan çalışmalarda hastaların ancak %30-40'ının tam düzelmeye ulaştığı, %60-70'inin depresif belirtilerinin devam ettiği, hatta bu hastaların %10-15'inin tamamen tedaviye yanıtsız kaldığı görülmüştür. Bunun başlıca nedeni, depresyon etiyolojisinin tam olarak aydınlatılmamış olmasıdır. TRP kanalları son 20-30 yıl içinde keşfedilmiştir. Yakın zamanda yapılan çalışmalarda birçok nörolojik ve psikiyatrik hastalığın etiyolojisinde bu kanalların rol oynadığı gösterilmesine rağmen TRPV1 kanalları ile depresyon arasındaki ilişkiyi araştıran çalışma henüz mevcut değildir. Agomelatin, son on yıl içinde keşfedilmiş antidepresan ilaçtır. MT1, MT2 ve 5-HT2c reseptör agonisti olup etki mekanizmaları kesin olarak bilinmemektedir. Bu çalışmada, 56 adet Wistar Albino cinsi 14-16 haftalık, deney başlangıcındaki ağırlıkları 255 ± 30 gram olan, erkek rat kullanılmıştır. Ratlar kronik hafif stres (KHS) ve kontrol grubu olarak iki gruba ayrılmıştır. Her iki grup da ilaç tedavisine göre (tedavi almayan, DMSO ya da agomelatin ile tedavi edilen) üç alt gruba ayrıldı. Depresyon modeli olarak, antidepresan etkiyi araştırmak için uygun olduğu düşünülen, yüksek geçerliliğe sahip KHS kullanılmıştır. Çalışmamızın sonucunda KHS modelinin ratlarda depresyon oluşturulmasında etkili olduğu sükroz tercih testi ile gösterilmiştir. Kronik hafif stres uygulanan grupta başlangıca kıyasla deneyin ilerleyen haftalarında giderek azalan sükroz tüketimi gözlenmiştir. Deneyin 4. haftasından itibaren verilen agomelatin tedavisinin ratlarda sükroz tüketiminde artışa neden olduğu saptanmıştır. Bu çalışma sonuçlarında, TRPV1 kanalları aracılığıyla sitozole Ca+2 girişinin depresyon etiyolojisinde rol oynadığı gözlenilmiştir. Bu nedenle, TRPV1 aracılı Ca+2 girişinin önlenilmesi ve buna yönelik ilaç geliştirilmesinin depresyonun neden olduğu apopitozu önlemede önemli olduğu gözükmektedir. Agomelatin, melatonin agonisti bir antidepresandır. Yakın zamanda yapılan çalışmalarda, TRP kanallarının inhibisyonunda melatonin etkisi olduğu bildirilmiştir. Bu tez çalışmasında da TRPV1 aracılı Ca+2 girişinin önlenilmesinde agomelatinin etkisinin olduğu literatürde ilk defa gösterilmiştir. Major depressive disorder that lifetime prevalence is stated %9-20, caused physical, mental, social, occupational disabilities which include recurrence, relapse and chronicity through phases. According to studies about drug efficiency in medical treatment %30-40 percentage of the patients completely overcome the disease however on the %60-70 percentage of the patients is still seen depressive symptoms furthermore %10-15 percentage of the patients do not response the medical treatment. The main reason of this situation is depression etiology is not identified completely. TRP channels have found out for 20-30 years. Even though in recent studies show that these channels take part on the etiology of many neurological and psychological disorders there is no study which seek the relationship between TRPV1 and depression yet. Agomelatine is an antidepressant drug which is discovered last ten years. Agomelatine is a receptor agonisf of MT1, MT2 and 5-HT2c however its mechanism of action is not known absolutely. In this study, 56 Wistar Albino 14-16 weeks, starting with the experimental weight 255 ± 30 g, were used in male rats. Rat chronic mild stress and the control group were divided into two groups. Both groups according to drug treatment (non-medicated, treated with DMSO or the agomelatine) were divided into three subgroups. As depression model to investigate the effects of antidepressants are thought to be appropriate, CMS with high validity is used. İn the results of our study, it indicated that the model of CMS is effective in creating depression on rats with the sucrose preference test. Chronic mild stress test was carried out in the coming weeks compared to baseline in the group's dwindling sucrose consumption was observed. Agomelatine treatment in rats given from 4 weeks of study was found to cause an increase in sucrose consumption. As a result of this study it has been shown that calcium' s entry to cytozole by TRPV1 channels has a role in depression etiology. The inhibition of calcium entry by TRPV1 and finding new drugs for that purpose looks to be important for stopping the apoptosis caused by depression. Agomelatine is a melatonin antagonist antidepressant. In a recent study it has been shown that melatonin has effect on inhibition of calcium' s entry by TRPV1. In this thesis study it has been first time shown in literature that agomelatine is effective in stopping calcium entry by TRPV1.
