Amaç: Bu çalışmamızda HFS'lilerde vestibüler sistemde etkilenme olup olmadığını, etkilenme varsa özelliklerini incelemeyi amaçladık. Materyal ve method: Çalışmaya nörolojik muayene ile HFS tanısı konulan 30 hasta ve kontrol grubuna 30 sağlıklı kişi dahil edildi. 30 HFS'li olgunun 22'si (%53,7) kadın ve 8'i (%42,1) erkekti. Yaş ortalaması 58,3 (±SD 9,9) olup, yaşlar 43 ila 69 arasında değişmektedir. Kontrol grubunun 19'u (%63,3) kadın ve 11'i (%36,6) erkekti. Yaş ortalaması 52,3 (±SD 13,2) olup, yaşlar 35 ile 70 arasında değişmektedir. Hastaların HFS süreleri, baş dönmesi, tinnitus, işitme kaybı vb kohleovestibüler yönden ayrıca sorgulandı. Hastalar idiyopatik veya postparalitik HFS'lilerden oluşmaktaydı. Hastalar botulinum nörotoksin (BoNT) uygulanarak takip edilen ve BoNT uygalanmadan takip edilenler olarak alt gruplara ayrıldı. Her olgunun anamnezi alınıp hastalara otoskopik muayene, baş boyun muayenesi ve nörolojik muayenede özellikle Fukuda, Romberg, göz açık Tandem ve göz kapalı Tandem yürüyüş testleri yapıldı. Anamnezde şikayet süresi, HFS tarafı, travma öyküsü, yüzdeki spazmı arttıran veya azaltan faktörler, polinöropati (PNP) ve distoni yönünden sorgulandı. Bulgular: Hasta ve kontrol grupları arasında boy, kilo ve yaş ortalamaları ile ilgili karşılaştırmada, yaş, kilo ve VKI bakımından anlamlı fark görülmezken, boy bakımından fark görüldü (P=0,01). HFS'li olgularda sağ ve sol tarafların etkilendiği olgu sayıları eşit olup 15 idi. Etyoloji yönünden 5 (%16,6) olgu postparalitik HFS iken, 25 (%83,3) olgu idyopatik HFS idi. Gruplar arasında etyoloji ve etkilenen taraf arasında karşılaştırma yapıldığında postparalitik HFS'li hastaların tümü sağ taraf etkilenmiş olgulardan oluşmaktaydı. Romberg testinde kontrol grubu ile hasta grubu arasında anlamlı fark buunmadı (p=0,83). Fukuda testinde HFS'lilerin 24 'ünde (%80) (15'i (%50) sola, 9'unda (%30) sağa) sapma varken, 6'sında (%20) sapma görülmedi. Kontrol grubunda Fukuda testinde tek olguda 10 derecelik sapma görüldü. Kontrol olgularında patolojik sapma görülmedi Fukuda testi sapma yönleri ile HFS etyoloji arasındaki ilişki anlamlı bulunmadı (p=0,468). BoNT uygulanan ve uygulanmayan hastalarda Fukuda Ortalama sapmaları yönünden anlamlı fark bulunmadı (p= 0,512). İlaç uygulanan ve uygulanmayan hastalar arasında da Fukuda Ortalama sapmaları yönünden anlamlı fark bulunmadı (p=0,543). İlaç ve BoNT uygulanan hastalar arasında Fukuda testi ortalama sapmaları arasında istatistiksel olarak anlamlı fark bulunmadı (p=0,641). Sonuç: HFS'li hastalarda Fukuda testinde ipsilateral 30 derece üstü sapmalar olduğu, Tandem gözü kapalı testin gözü açık teste göre bozuk olduğu görüldü. Bu bulgular idiyopatik HFS'lilerde de postparaltik HFS'lilerde olduğu gibi periferik sinir etkilenmesinin temel mekanizma olduğu görüşünü desteklemenin yanı sıra HFS'li hastalarımızda statik denge testlerinden olan (Romberg testinde) anlamlı bozukluk görülmezken; dinamik denge testlerinde (Fukuda testinde, göz açık Tandeme göre göz kapalı Tandem testi bozuk olanlar) bozukluğun anlamlı düzeyde korole olduğunu gördük.Bunu HFS'li olgularda vestibüler sistem etkilenmesinin de olduğu lehine yorumladık. In this study we aimed to investigate wheter the vestibular system is attached basically in patient with HFS and if so the characteristic of the vestibular system. The HFS durations of the cases were also research in the cohleovestıbular directions sech as especially dizness, tinnitus, hearing loss etc. Idiopatic and post paralytic HFS were seperated a group of botulinum neurotoxin (BoNT) was applied and the group was followed up whithout BoNT applications. In every case anamnesis was taken and findings were recorded by performance otoskopic examination and neurologycal examination especially Fukuda, Romberg, eyes open and closed Tandem walking test of the patients. The history of complaints at the anamnesis was researched by the HFS side, trauma story factors that increase or decrease the spasm in the face additional discases (Polynoropathy, diystonia). Findings: While there was no significant difference in wright, age and BMI and the control groups there was a different in height (p=0,24). In the cases with HFS which was affected right and left sides were the same and it was 15. 5 (16,6%)cases were postparalytic HFS due to etiology and 25 (83,3%) cases were idiopatic HFS. When the etiology and the affected sıde were compared between the groups all of the patients with postparalytic HFS have consisted of right side affected patients. In the Fukuda test, 15 (50%) of the cases were deviated to the left, 9 (30%) were deviated to the right. 6 of the cases weren't observed the deviation. The relationship between the deviation directions of Fukuda test and HFS etiology was evoluated and there was no significant (p=0,48). Eyes open Tandem test for head trauma HFS cases was in computent in 1 (12,5%) of the 8 patients. Eyes closed Tandem test was ineffective in 3 (37,5%) patients. In the cases of the head trauma story in HFS cases both eyes open Tandem test inability(p=0,93) and eyes closed tandem test inability (p=0,12) weren't found significantly different from the not head trauma story. Fukuda average deviations were avoluted in patients applied BoNT (p=0,51). Fukuda average deviations were evoluated in patient applied medicine (p=0,543). There was no statistically difference between the general deviations of the Fukuda test and the patients who were applied with medicine and BoNT. Conclusion: In HFS patient it was seen that eyes closed Tandem test was distorted to the eyes open test. In Fukuda test ipsilateral 30 degrees above were deviated. These fİndİngs support the wiev thet peripheral nerve involvment is the main mechanism of action in idiopatic HFS as well as in postparalytic HFS. In patients with HFS while static balance tests (Romberg) have been disordered, dynamic balance tests have been significantly disordered we have interprated this in faver of the vestibular system effect in HFS cases.
