Prematüre doğumlar gerçekleşen tüm doğumların %12'sini oluşturmaktadır. Prematüre doğum oranı artmakla birlikte gelişen yenidoğan yoğun bakım ünitesi koşulları, müdahalede bulunan ekip eğitimlerinin tamamlanması, erken antenatal önlemlerin alınması ile prematüre yenidoğan yaşam oranları artmıştır. Vitamin D tüm vücutta bulunan yaygın reseptörleri vasıtasıyla 1.000'den fazla gen üzerindede etkili olduğunun saptanması nedeniyle artık bir vitamin olarak değil bir hormon olarak tanımlanmaktadır (1). Çalışmamızın amacı prematüre bebeklerde, vitamin D düzeyinin neonatal morbiditeler (BPD, sepsis, NEK, IKK, RDS, ROP, osteopeni) üzerine etkisini araştırmaktı. Bu çalışma Süleyman Demirel Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları, Yenidoğan Yoğun Bakım Ünitesi'nde 04.11.2015-04.11.2016 tarihleri arasında kesitsel olarak yapıldı. Prospektif olarak yapılan çalışmada 37 gebelik haftası öncesinde doğan ve immatürite nedeniyle neonatal morbiditelerin gelişme riski yüksek olan 80 prematüre bebek alındı. Anneler için demografik özellikleri tanımlayıcı anketler düzenlenerek anket cevapları kaydedildi ve ailelerden bilgilendirilmiş onam formu alındı. Hastaların prenatal, natal ve postnatal bilgileri hasta dosyalarından ve takip kayıtlarından elde edildi. Çalışmaya alınan bebeklerin 44'ü erkek (%55), 36'sı kız (%45)'dı. Tüm bebeklerden doğum esnasında kord kan örnekleri alınarak 25 hidroksi vitamin D düzeyleri ölçüldü. Çalışmaya alınan bebeklerin kord kanı 25(OH)D3 düzeyleri 3,00 ile 45,50 ng/mL (ortalama değeri 15,76±9,44 ng/mL) arasındaydı. Vitamin D düzeyi sınıflamasına göre 14 (%17,5) hastada ağır eksiklik, 16 (%20) hastada orta eksiklik, 13 (%16,3) hastada hafif eksiklik, 5 (%6,2) hastada yetmezlik, 32 (%40) hastada 25(OH)D3 düzeyleri normal aralıklarda tespit edildi. Çalışmaya alınan prematüre yenidoğanların 40'ında (%50) RDS, 13'ünde (%16,2) İKK, 24'ünde (%30) ROP, 37'sinde (46,3) sepsis, 28'inde (35) osteopeni ve 29'unda (%36,2) BPD gözlendi. Prematüre bebeklerde vitamin D düzeyinin yenidoğan morbiditeleri üzerine etkisini araştıran klinik çalışmamızda vitamin D eksikliği olan prematüre yenidoğanlarda BPD, sepsis, RDS, NEK, İKK, osteopeni, ROP görülme sıklığının arttığı ve istatistiksel olarak anlamlı olduğu tespit edildi. Bu nedenle gebe kadınlarda uygun dozda vitamin D takviyesi yapılması ve gebelik takibinde vitamin D serum değerinin yakından izlenmesinin yenidoğan morbiditelerini önlemeye katkıda bulunabileceğini düşünmekteyiz. Vitamin D eksikliği olan anneden doğan bebeklerde hem yenidoğan döneminde, hem çocukluk yaşlarında ciddi sağlık sorunları olması nedeniyle özellikle gebelik döneminde vitamin D düzeyleri yakın takip edilip eksiklikler tedavi edilmeli, normal olanlara profilaktik destek mutlaka verilmelidir. Çalışmamızda elde ettiğimiz verilerde dikkate alınarak gebe kadınlara uygun dozda Vitamin D takviyesi yapılması ve serum değerlerinin izlenmesinin, yenidoğan morbiditelerini azaltmak için basit, ucuz ve etkili bir metod olabileceğini düşünmekteyiz. The aim of our study was to investigate the influence of vitamin D level on neonatal morbidities (BPD, sepsis, NEC, ICH, RDS, ROP, osteopenia) in premature babies. This study has been conducted as a cross-sectional study at Suleyman Demirel University Medical Faculty Department of Pediatrics, Newborn Intensive Care Unit, between 04/11/2015 and 04/11/2016. This prospective study has been conducted on 80 premature babies who comply with the working criteria, which are being born before 37th pregnancy week and having a high risk of neonatal morbidities due to immaturity. Surveys which define the demographic characteristics of the mothers have been conducted and the responses to the surveys have been recorded and informed consent forms have been taken from the families. The prenatal, natal, and postnatal data of the patients have been taken from the patient files and follow-up records. Of all the babies in the study, 44 were male (55%) and 36 were female (45%). 25-Hydroxyvitamin D levels of the babies were measured by taking cord blood samples of all the babies during birth. The cord blood 25(OH) vitamin D levels of the babies in the study were in between 3,00 and 45,50 ng/mL (with a mean of 15,76 ± 9,44 ng/mL). According to the vitamin D level classification, heavy deficiency in 14 (17%) patient, medium deficiency in 16 (20%) patient, mild deficiency in 13 (16,3%) patient, insufficiency in 5 (6,2%) patient have been found. 32 (40%) patients have been found to be within the normal range of 25(OH) vitamin D. It is observed that, of the premature newborns in the study, 40 (50%) had RDS, 13 (16,2%) had ICH, 24 (30%) had ROP, 37 (46,3%) had sepsis, 28 (35%) osteopenia, and 29 (36,2%) had BPD. In our clinical study which investigates the influence of vitamin D levels in premature babies on newborn morbidities, it is found that the frequency of BPD, sepsis, RDS, NEC, ICH, osteopenia, and ROP is increased in premature newborns who have vitamin D deficiency and the increase is found to be statistically significant. For this reason, we think that vitamin D supplement with appropriate doses in pregnant women and watching the serum value of vitamin D in pregnancy follow-ups can contribute to the prevention of newborn morbidities. Because, both in newborn period and in childhood ages, there are serious health problems in babies of mothers who have vitamin D deficiency, vitamin D levels should be followed closely especially during pregnancy, deficiencies should be treated, and if they are normal, prophylactic support should certainly be supplied. Taking into account the data we obtained in our study, we think that vitamin D supplement with appropriate doses to pregnant women and watching the serum value can be a simple, cheap, and efficient method for decreasing newborn morbidities.
