Alkalen reflü; çeşitli sebeplerle duodenumdaki safra, pankreas ve ince barsak sekresyonlarının mideye geri gelmesi, alkalen reflü gastrit ise bu reflüye bağlı mide mukozasında oluşan zedelenmedir. Alkalen reflü bazen özofagusa kadar ulaşıp duodeno-gastro-özefaeal reflü olarak karşımıza çıkar. Safra tuzlarının deterjan etkisi nedeniyle özofagus ve mide mukozasında hasara neden olur. Safra reflüsü sonucunda özofajit, barret özofagus, gastrit ve intestinal metaplazi görülebilir. Ayrıca yapılan çalışmalarda safra reflüsünün H. Pylori pozitifliği ile negatif bir ilişkinin söz konusu olabileceği saptanmıştır. Duodenit ile alkalen reflü arasında korelasyon bulunmaktadır. Kolesistektomili hastalarda da alkalen reflü gastrit daha sık bulunmuştur. Çalışmamızda primer alkalen reflü gastrit ile endoskopik ve histolojik değişiklikler, safra yolu patolojileri ile ilişkisi, anemi ve karaciğer fonsiyon testleri ile ilişkisinin değerlendirilmesi amaçlandı. Çalışmaya 05.2015-05.2016 tarihleri arasında endoskopi yapılmış olan hastalardan kendi belirlemiş olduğumuz kriterlere uygun endoskopik olarak 115 alkalen reflü gastriti olan ve 98 alkalen reflü gastrit olayan 213 hasta alındı. Hastaların endoskopik görünümleri, histopatolojik sonuçları, safrayolu patolojileri, anemi parametreleri ve karaciğer enzimleri retrospektif olarak değerlendirildi. Araştırmanın sonucunda kolesistektomi ile safra reflüsü arasında anlamlı bir ilişki saptandı. Endoskopik değerlerlendirmede alkalen reflü grubunda pangastrit daha yüksek bulunurken, duodenuma ait daha az patoloji izlendi. Histopatolojik incelemede ise alkalen reflü grubunda kronik gastrit daha sık bulunmasına rağmen istatistiksel olarak anlamlı değildi. Özofagus patolojileri, duodenit, H. Pylori, atrofi, intestinal metaplazi açısından anlamlı fark saptanmadı. Laboratuvar değerlendirmede alkalen reflü grubunda Hb, ferritin, transferrin saturasyonu, ALT ve AST daha yüksek bulundu. Sonuç olarak; alkalen reflü gastritin midede kronik inflamasyonu arttırdığı ve pangastrite neden olduğu görüldü. Kolesistektominin safra reflüsü ile ilişkili olduğu görüldü. AST ve ALT düzeyleri alkalen reflü gastrit grubunda yüksek bulundu. Ancak bu konuda daha ayrıntılı ve daha fazla hasta içeren çalışmalara ihtiyaç olduğu düşünüldü. Primer duodenogastrik reflü üzerine çalışmaların azlığı nedeniyle bu konuya yönelik daha çok çalışma yapılabilir. Alkaline reflux is which bile, pancreas and intestinal secretions in the duodenum come back to the stomach with various reasons and alkaline reflux gastritis is injury in the gastric mucosa because of this reflux. Alkaline reflux sometimes reaches to the esophagus and comes out as duodeno-gastro-esophageal reflux. Bile salts cause damage to the esophagus and stomach mucosa due to the detergent effect. Esophagitis, barrette esophagus, gastritis and intestinal metaplasia may be seen as a result of biliary reflux. In addition, studies have shown that bile reflux may have a negative relationship with H. pylori positivity. There is a correlation between duodenitis and alkaline reflux. It is found that alkaline reflux gastritis is more common in cholecystectomy patients. In our study, we aimed to evaluate the relationship between primary alkaline reflux gastritis and endoscopic and histological changes, relation with biliary pathology, anemia and liver function tests. We enrolled totally 213 patients according to our criteria who was applied endoscopy at between 05.2015-05.2016, 115 of them with alkaline reflux gastritis and 98 of them with no alkaline-reflux gastritis. Endoscopic appearance, histopathological results, pathologic pathology, anemia parameters and liver enzymes were evaluated retrospectively. As a result of the study, there was a significant correlation between cholecystectomy and biliary reflux. It was found that pangastrit was higher and duodenal mucosal pathology was less in alkaline reflux group at endoscopic evaluation. In the histopathological examination, although chronic gastritis was more common in the alkaline reflux group, it was not statistically significant. There was no significant difference in terms of esophageal pathologies, duodenitis, H. pylori, atrophy, intestinal metaplasia. At the laboratory evaluation; Hb, ferritin, transferrin saturation, ALT and AST were higher in the alkaline group. As a result, it was seen that alkaline reflux gastritis incerased chronic inflammation and caused pangastritis. It was seen that cholecystectomy was associated with biliary reflux. AST and ALT levels were higher in the alkaline reflux gastritis group. However, more detailed studies involving more patients were needed in this subject. Due to the lack of studies on primary duodenogastric reflux, more studies can be done on this topic.
