Yarık Damak-Dudak Tanılı Ergenlerde Psikiyatrik Komorbidite, Psikolojik Profilin Belirlenmesi ile Problemli İnternet Kullanımı ve Geleneksel-Siber Zorbalık-Mağduriyet İlişkisinin İncelenmesi Amaç: Bu çalışmada yarık deformiteli ergenlerin psikiyatrik komorbidite ile benlik saygısı, sosyal görünüş kaygısı, yalnızlık düzeyleri, problemli internet kullanımı, geleneksel ve siber zorbalık-mağduriyet düzeyleri açısından kontrol grubuyla karşılaştırmalı olarak değerlendirilmesi amaçlanmıştır. Yöntem: Araştırma Süleyman Demirel Üniversitesi Plastik, Rekonstüktif ve Estetik Cerrahisinde yarık deformitesi tanısı ile takipli olan 40 yarık deformiteli ergen ile yaş ve cinsiyet açısından olgu grubuyla birebir eşleştirilmiş 40 sağlıklı kontrol ile yürütülmüştür. Olgu ve kontrol gruplarına Piers-Harris Benlik Saygısı Ölçeği (PHBSÖ), Sosyal Görünüş Kaygısı Ölçeği, UCLA Yalnızlık Ölçeği Kısa Formu, Problemli İnternet Kullanımı Ölçeği, Siber Zorbalık ve Mağduriyet Ölçeği, Akran Zorbalığını Belirleme Ölçeği uygulanmıştır. Her iki grup Okul Çağı Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Şekli Türkçe uyarlaması ile değerlendirilmiştir. Ayrıca olgu grubuna Cleft-Q Yaşam Kalitesi Ölçeği uygulanmıştır. Sonuçlar: Yarık deformiteli ergenlerin kontrollere kıyasla sosyal görünüş kaygılarının daha yüksek olduğu saptanmıştır. Yarık deformiteli ergenlerin boş zamanlarında en çok ve kontrollerden anlamlı oranda daha fazla internette vakit geçirdiği saptanmıştır. Yarık deformiteli ergenlerin internette aile denetimlerinin anlamlı oranda fazla saptanması bu ergenleri internetin olmsuz sonuçlarından korumuş olabilir. Yarık deformiteli ergenlerde ruhsal tanı varlığının kontrol grubuna göre daha yüksek oranda görüldüğü ve bu farkın anlamlı olduğu saptanmıştır. Yarık deformiteli ergenlerde en sık saptanan tanı sosyal fobi yaygın olup kontrol grubuna göre anlamlı oranda daha fazla görülen diğer ruhsal tanı ise enuresis noktürnadır. Tartışma: Çalışmamızdaki yarık deformiteli ergenlerde psikiyatrik komorbidite ve sosyal görünüş kaygısının yüksek oranlarda saptanması yarık deformitesi tedavi planında psikiyatrik değerlendirmeye yer verilmesinin gerekliliğini düşündürmüştür. Bu çalışma ile yarık deformitesi hastalarının tedavisinde multidisipliner yaklaşımın önemi ortaya çıkmaktadır. The Determination of Psychiatric Comorbidity, Psychological Profile and Examination of the Relationship Between Problematic Internet Usage and Traditional-Cyber Bullying-Victimization in Adolescents with Cleft Lip and Palate Objectives: The aim of this study was to assess psychiatric comorbidity and levels of self-esteem, social appearance anxiety, loneliness, problematic Internet usage, traditional and cyber bullying-victimization in adolescents with cleft lip and palate as compared with a control group. Method: The study consisted of 40 adolescents who followed with diagnosis of cleft lip and palate in Plastic, Reconstructive and Aesthetic Surgery of Süleyman Demirel University and 40 healthy controls paired with each other in terms of age and gender. The Piers-Harris Self-Esteem Scale (PHBSÖ), Social Appearance Anxiety Scale, UCLA Loneliness Scale Short Form, Problematic Internet Usage Scale, Cyber Bullying and Victimization Scale and Peer Bullying Identification Scale were administered to the case and control groups. Both groups were assessed according to the Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version-Turkish Version. Additionally, the Cleft-Q Quality of Life Scale were administered to the case group. Results: It was found that the social appearance anxiety of cleft deformed adolescents were higher than controls. It has been found that cleft deformed adolescents spend most of their in spare time on the Internet and this is more significantly than controls. The fact that there are a lot of family controls on the internet of cleft deformed adolescents on the internet can protect these adolescents from their uninformed results. The presence of psychiatric diagnosis in cleft deformed adolescents was found to be higher in comparison with the control group, and this difference was found to be significant. Social phobia is the most common diagnosis in cleft deformed adolescents and enuresis nocturna is the other psychological diagnosis which is more significant than control group. Discussion: The presence of high levels of psychiatric comorbidity and social appearance anxiety in adolescents with cleft lip and palate in our study suggests that psychiatric evaluation should be included in cleft lip and palate treatment plans. Our study demonstrates the importance of a multi-disciplinary approach for cleft lip and palate treatment.
