GİRİŞ VE AMAÇ Günümüzde yağ dokusundan elde edilen kök hücreler üzerinde bir çok çalışma yapılmaktadır. Yağ greftlerinin ucuz, bol, kolay elde edilebilen ve özellikle enjekte edilebilir bir doğal dolgu olarak kullanılabilmesi yakın geçmişte bu alana yoğunlaşmış bir ilgi uyandırmıştır. Adipositlerden elde edilen SVF kolay elde edilebilirliği ve içeriğinde bulundurduğu hücre sayısının fazla olması nedeniyle mezenkimal kök hücre için ideal kaynak olmuştur. Bir çok faktör yağ hücre kaynaklı kök hücrelerin rejenaratif potansiyelinin altında yatmaktadır. Bunlar: (a) yağ grefti olarak transfer edilmiş hücrelerin etrafında bulunan yağ hücre kaynaklı kök hücreler iskemiye normal adipositlerden daha dayanıklıdır. (b) yağ hücre kaynaklı kök hücreler anjiojenik özellik taşımaktadır ve iskemi esnasında anjiojenik faktörleri salmaktadır. (c) yağ hücre kaynaklı kök hücreler yağ hücrelerine transfer edildiğinde greft resorpsiyonuna etki etmektedir. Yağ greftleri, SVF/ADSC veya SVF/ADCS ile zenginleştirilmiş yağ grefti uygulamaları, ister rejeneratif-tedavi edici ister estetik-rekonstrüktif amaçlarla yapılsın istenilen sonuca ulaşmak için tekrarlayan seanslar gerektirmektedir. Her uygulamada yağ alma gerekliliği nedeniyle maliyet ve emek artışı, ameliyathane şartlarını sağlama zorunluğu, hastanın yaşantısını etkileme ve işten uzak bırakma gibi çekinceler tekrarlayan uygulamaları zorlaştırmaktadır Tekrarlayan yağ alımından kurtulmak için akla ilk gelen çözüm yağ dokusu için güvenilir saklama koşulları oluşturmaktır. Dondurarak saklama ilk akla gelen yöntemdir. Çalışmanın amacı yağ greftleri, taze yağ greftlerinden mekanik olarak elde edilen SVF'ler ve dondurulup çözdürülmüş yağ greftlerinden mekanik olarak elde edilen SVF'lerde çeşitli saklama sürelerinde dondurma sıcaklığı ve koruyucu madde kullanımının canlılığa etkisinin araştırılmasıdır. Materyal method: Liposuction ile elde edilen insan kaynaklı yağ greftleri ve SVF'ler dondurulup çözdürülerek taze yağ greftleri ve SVF'ler ile karşılaştırıldı. Karşılaştırmalar 0. gün, 3. ay ve 9. ayda örnekler çözdürülerek yapıldı. Her çalışma dönemi için -20 °C, -80 °C ve -196°C şeklinde 3 farklı saklama sıcaklığı test edildi. Ayrıca dondurarak saklanmış yağ greftlerinden aynı mekanik yöntemle SVF elde edildi, dondurularak saklamış yağ greftlerinin SVF eldesi için uygunluğu test edildi, ayrıca bu şekilde elde edilmiş SVF'ler dondurularak saklanmış SVF'ler ile karşılaştırıldı. Dondurma işlemi; Dimetil Sulfoksitin kullanıldığı (DMSO +) ve kullanılmadığı (DMSO -) iki farklı ortamda yapıldı. Taze veya dondurulup çözdürülmüş yağlardan mekanik yolla elde edilen SVF'lerin canlılıkları, içerdikleri canlı hücre miktarları tripan mavisi ile boyanmış örnekler thoma lamında sayılarak incelendi. Yağ greftlerinin canlılık incelemesi histokimyasal (Hematoksilen & Eozin (H&E) boyama ile yapıldı. 9. ay yağ greftlerinde yeterli hücresel canlılık elde edildikten sonra canlılığı en yüksek -196°C DMSO+ saklama grubundan elde edilen örnekler immünbaskılanmış sıçan modeline verilerek 3. ayda taze örnekler ile elde ettiğimiz sonuçlar ile karşılaştırıldı. resultUzun süreli saklama gruplarında en iyi sonuç veren sıcaklık ve koruyucu kullanım etkisi olan -196°C'de DMSO+ yağ greftleri, in vivo olarak da 9 ay sonunda en fazla canlılığa sahip immunbaskılanmış sıçanlara verilerek test edildi. Çalışmamızın bir yıl içerisinde deneysel ve hayvansal kısmı tamamlandı. Çalışmamızın klinik ayağında ise -196°C'de dondurulmuş DMSO+ yağ greftleri seri enjeksiyonlar halinde hastalara verildi. Uzun süreli saklamanın SVF'ler ve yağ greftleri için mümkün olduğu izlendi. Sonuç olarak düşük sıcaklıklarda koruyucu madde ile saklanan SVF'ler ve yağ greftleri canlılığını büyük oranda korumaktadır. Dondurularak saklanan yağ greftleri sayesinde hasta yeniden operasyon geçirmeye ihtiyaç duymamakta, hem cerrah hem hasta için büyük kolaylık sağlanmaktadır. Çalışmamızda görüldüğü üzere düşük sıcaklık ve koruyucu madde ile saklanan yağlar ve SVF'ler canlılığını 9 ay süre ile muhafaza etmektedir. Bu çalışmadan yola çıkarak ve hücrelerin canlılığını koruduğu göz önüne alınarak, dondurulmuş ve saklanmış yağ grefti ve SVF'lerin klinik uygulama aşamasında daha farklı alanlarda kullanılmasına ihtiyaç olduğu ve faydalı olacağı düşünülmektedir.

SVF obtained from adipocytes has been an ideal source for mesenchymal stem cells due to its easy availability and the high number of cells it contains. Many factors underlie the regenerative potential of adipose cell-derived stem cells. These are: (a) adipose cell-derived stem cells found around cells transferred as fat graft are more resistant to ischemia than normal adipocytes. (b) adipose cell-derived stem cells have angiogenic properties and release angiogenic factors during ischemia. (c) adipose cell-derived stem cells affect graft resorption when transferred to fat cells. Fat grafts, SVF/ADSC or SVF/ADCS-enriched fat graft applications, whether for regenerative-therapeutic or aesthetic-reconstructive purposes, require repetitive sessions to achieve the desired result. Restrictions such as cost and labor increase due to the necessity of removing fat in each application, the necessity of providing the operating room conditions, affecting the patient's life and leaving them away from work make repetitive applications difficult. The first solution that comes to mind to get rid of repetitive fat intake is to create reliable storage conditions for adipose tissue. Freezing is the first method that comes to mind. The aim of the study is to investigate the effects of freezing temperature and use of preservatives on viability at various storage times in fat grafts, SVFs obtained mechanically from fresh fat grafts and SVFs obtained mechanically from frozen and thawed fat grafts. Material method: Human-derived fat grafts and SVFs obtained by liposuction were frozen and thawed and compared with fresh fat grafts and SVFs. Comparisons were made by thawing the samples at day 0, 3, and 9 months. For each working period, 3 different storage temperatures were tested as -20 °C, -80 °C and -196 °C. In addition, SVF was obtained from cryopreserved fat grafts with the same mechanical method, the suitability of cryopreserved fat grafts for SVF extraction was tested, and SVFs obtained in this way were compared with freeze-preserved SVFs. Freezing process; It was done in two different environments with (DMSO +) and without (DMSO -) dimethyl sulfoxide. The viability of SVFs obtained mechanically from fresh or frozen-thawed oils and the amount of viable cells they contain were examined by counting the samples stained with trypan blue on a thoma slide. Viability analysis of fat grafts was performed by histochemical (Hematoxylin & Eosin (H&E) staining. After sufficient cellular viability was obtained in the 9th month fat grafts, the samples obtained from the -196°C DMSO+ storage group with the highest viability were given to the immunosuppressed rat model and fresh samples were added at the 3rd month. compared with our results. DMSO+ fat grafts at -196°C, which has the best results in long-term storage groups and the effect of temperature and protective use, were tested in vivo by giving them to immunosuppressed rats with the highest viability at the end of 9 months. The experimental and animal part of our study was completed within a year. In the clinical part of our study, DMSO+ fat grafts frozen at -196°C were given to the patients as serial injections. It has been observed that long-term storage is possible for SVFs and fat grafts. As a result, SVFs and fat grafts stored with preservatives at low temperatures maintain their vitality to a large extent. Thanks to the fat grafts stored by freezing, the patient does not need to undergo reoperation, providing great convenience for both the surgeon and the patient. As seen in our study, oils and SVFs stored at low temperature and with preservatives maintain their vitality for 9 months. Based on this study and considering that the cells maintain their viability, it is thought that frozen and stored fat grafts and SVFs need to be used in different areas during clinical application and it will be beneficial.
