Giriş Amaç: Perkütan böbrek biyopsisi (PBB) renal hastalıkların tanısında ve yönetiminde kullanılan, güvenli bir yöntemdir. Bu işlemin ultrasonografi (USG) eşliğinde yapılması ve otomatik biyopsi iğnelerinin kullanılmasına bağlı olarak alınan doku yeterliliği ve işlemin güvenilirliği artsa da %100 başarı sağlanamamıştır. Bu işlem koaksiyel ve nonkoaksiyel biyopsi yöntemleriyle yapılabilir. İşlemle ilişkili en sık karşılaşılan komplikasyon kanama olup bu asemptomatik olabileceği gibi yaşamı tehdit eden ciddi kanamalara neden olabilir. Bu çalışmadaki amacımız görüntüleme eşliğindeki koaksiyel ve nonkoaksiyel yöntem kullanılarak yapılan PBB'de; tanı koymadaki glomerül sayısı bakımından doku yeterliliğinin ve işleme ait komplikasyonların karşılaştırılmasıdır. Ayrıca bu yöntemlerle alınan glomerül miktarındaki skleroze glomerül sayısının komplikasyon gelişmesine etkisi araştırmayı amaçladık. Gereç-Yöntem: Bu çalışmada Ocak 2013 -Aralık 2021 tarihleri arasında Girişimsel Radyoloji kliniğimize renal parankim hastalığı ön tanısı ile yönlendirilerek USG eşliğinde koaksiyel ve nonkoaksiyel yöntemle PBB yapılmış hastaların bilgileri tarandı. Hastaların demografik verileri, laboratuvar bulguları, işlem öncesi USG'de renal parankim kalınlığı ve ekojenitesi, histopatolojik renal biyopsi sonuçları, alınan materyaldeki glomerül sayısı, materyaldeki sklerotik glomerül sayısı, biyopsi sonrası komplikasyon gelişme durumu hastane kayıtlarından elde edildi. Hastalar pron pozisyonunda yatırılarak uygun steril ortam sağlanıp lokal anestezi altında USG eşliğinde serbest el tekniği ile biyopsi işlemi uygulandı. Her iki yöntemde de 16 G otomotik biyopsi tabancası kullanıldı. Nonkoaksiyel yöntemde; biyopsi iğnesi böbrek alt pol korteksine ultrason eşliğinde gerçek zamanlı olarak ilerletildi ve iğne kapsüle ulaşıldıktan sonra, ekojen renal hilus korunacak şekilde ateşleme yapıldı. Her parça için işlem aynı şekilde tekrarlandı. Koaksiyel yöntemde ise koaksiyel iğne böbrek korteks kapsülüne geldiğinde mandreni çıkarılıp içerisinden otomatik biyopsi iğnesi geçirilerek aynı işlem yapıldı. Her parça için kılavuz iğne aynı yerde sabit bırakılıp otomotik biyopsi tabancası içerisinden geçirilerek işlem yapıldı. Her hastada patolog tarafından glomerül yeterliliği onayı alınarak 1-4 kez işlem tekrarlandı. İşlem bitiminde ve 4 saat sonra Hb düşüşü olan hastalarda USG yapılıp komplikasyon gelişip gelişmediği kaydedildi. Her iki yöntemde alınan biyopsilerde spesifik tanı koyulabilme açısından karşılaştırma yapıldı. Bunun yanısıra her iki yöntemde gelişen komplikasyonların parça sayısı ve sklerotik glomerül sayısı ile ilişkisi analiz edildi. Bulgular: İşlem yaptığımız 239 hastanın 145'ine nonkoaksiyel yöntem, 94'üne ise koaksiyel yöntem uygulandı. Nonkoaksiyal grupta 129 (%89,0) hastaya tanı koyulabilirken, koaksiyel grupta 92 (%97, 9) hastanın tanısı koyulabildi. Koaksiyel yöntemle patolojik tanı koyma oranı daha fazla olarak tesbit edildi. Yapılan işlemlerimizde sadece kanama komplikasyonu gelişti. Gelişen kanama 1 cm altı ve 1 cm üstü perirenal hematom olarak iki gruba ayrıldı. Nonkoaksiyel yöntem uygulanan hastaların 120'sinde komplikasyon gelişmemişken; 21'inde 1 cm altı, 4'ünde ise 1 cm üstü perirenal hematom görüldü. Koaksiyel yöntem uygulanan hastaların 90'ında komplikasyon gelişmemişken; 1'inde (%1,1) 1 cm altı, 3'ünde (%3,2) ise 1 cm üstü perirenal hematom görüldü. Skleroze glomerül sayısı ile komplikasyon arasında anlam saptanmadı. Sonuç: Koaksiyel teknik, nonkoaksiyel tekniğe göre daha düşük kanama komplikasyon oranı ve tanısal yeterlilik açısından daha yüksek bir orana sahipti. Although the adequacy of the tissue taken and the reliability of the procedure increased due to the fact that this procedure is accompanied by ultrasonography (USG) and the use of automatic biopsy needles, 100% success has not been achieved. This procedure can be done with coaxial and noncoaxial biopsy methods. The most common complication associated with the procedure is bleeding, which can be asymptomatic or cause serious life-threatening bleeding. Our aim in this study is in PBB performed using coaxial and noncoaxial methods accompanied by imaging; It is the comparison of tissue adequacy in terms of the number of glomeruli at diagnosis and the complications of the procedure. In addition, we aimed to investigate the effect of the number of sclerosed glomeruli on the amount of glomeruli removed by these methods on the development of complications. Material-Method: In this study, the information of patients who were referred to our Interventional Radiology clinic with a preliminary diagnosis of renal parenchyma disease and underwent PBB with coaxial and noncoaxial method under USG guidance, between January 2013 and December 2021 were scanned. Demographic data of the patients, laboratory findings, renal parenchyma thickness and echogenicity on USG before the procedure, histopathological renal biopsy results, number of glomeruli in the material taken, number of sclerotic glomeruli in the material, post-biopsy complication development status were obtained from hospital records. The patients were placed in the prone position and a suitable sterile environment was provided, and biopsy was performed under local anesthesia with the free hand technique under the guidance of USG. 16 G automatic biopsy gun was used in both methods. In the noncoaxial method; The biopsy needle was advanced to the lower pole cortex of the kidney under ultrasound guidance in real time, and after the needle was reached in the capsule, firing was done so that the echogenic renal hilus was preserved. The process was repeated in the same way for each piece. In the coaxial method, when the coaxial needle came to the renal cortex capsule, the chuck was removed and an automatic biopsy needle was inserted through it, and the same procedure was performed. For each part, the guide needle was left fixed inside and the procedure was performed by passing it through the automatic biopsy gun. In each patient, the procedure was repeated 1-4 times after obtaining glomerular adequacy approval by the pathologist. At the end of the procedure and 4 hours later, USG was performed in patients with Hb decrease and it was recorded whether complications developed or not. Biopsies taken in both methods were compared in terms of making a specific diagnosis. n addition, the relationship between the number of fragments and the number of sclerotic glomeruli of the complications developed in both methods was analyzed. Results: The noncoaxial method was applied to 145 of the 239 patients we treated, and the coaxial method was applied to 94 of them. While 129 (89.0%) patients could be diagnosed in the noncoaxial group, 92 (97.9%) patients could be diagnosed in the coaxial group. The rate of pathological diagnosis was found to be higher with the coaxial method. Only bleeding complication developed in our procedures. The bleeding that developed was divided into two groups as perirenal hematoma below 1 cm and above 1 cm. While there was no complication in 120 of the patients who underwent the noncoaxial method; Perirenal hematoma was observed below 1 cm in 21 patients and above 1 cm in 4 patients. While 90 of the patients who underwent coaxial method did not develop complications; Perirenal hematoma was observed less than 1 cm in 1 (1.1%) and over 1 cm in 3 (3.2%) patients. There was no significant difference between the number of sclerosed glomeruli and complications. Conclusion: The coaxial technique had a lower bleeding complication rate and higher diagnostic efficiency than the noncoaxial technique.
