Amaç: Çalışmamızın amacı Süleyman Demirel Üniversitesi Gastroenteroloji Bilim Dalı'nda takip edilen OİH hastalarının özellikleri ve tedaviye yanıtı etkileyen noninvaziv ve invaziv faktörlerin araştırılmasıdır. Gereç ve Yöntem: Çalışmamız için Süleyman Demirel Üniversitesi Araştırma ve Uygulama Hastanesi Gastroenteroloji Bilim Dalında takip edilen 71 OİH tanılı hastanın verileri retrospektif olarak incelemeye alındı. Tedaviye yanıt ile noninvaziv ve invaziv faktörlerin korelasyonu istatistiksel olarak hesaplandı. Bulgular: Çalışmamıza katılan 71 hastanın %74,6'sı kadın, %25,4'ü erkekti. Hastaların ortalama yaşı 53,38 ± 15,08'di. Tanı anında ortanca yaş 48'di. Hastaların %32,4'ünde ekstrahepatik otoimmün hastalık vardı, en sık otoimmün tiroid hastalıkları görüldü. Hastaların %25,3'ünde overlap sendromu görüldü. OİH-PBK overlapı (%23,9) OİH-PSK overlapına (%1,4) göre daha sık saptandı. En sık başvuru şikayetleri, sarılık(%21,1), halsizlik-yorgunluk(%15,5) ve karın ağrısıydı(%11,3). Hastaların %81,7'si tip 1 OİH, %1,4'ü tip 2 OİH, %15,5'u otoantikor negatif OİH olarak değerlendirildi. Hastaların %42,3'ünde tanı anında siroz mevcuttu. En sık görülen ultrasonografik bulgu, kronik karaciğer hastalığı bulguları(%43,7) ve hepatosteatozdu(%31). Steroid ve azatiyopürin içeren kombinasyon tedavisi monoterapiye göre daha çok tercih edildi(%95,8'e karşı %2,8). Hastaların %45,1'inde remisyon, %53,5 hastada tedaviye eksik yanıt ve %1,4 hastada tedavi başarısızlığı gözlendi, %46,9 hastada nüks görüldü. İzole OİH tanılı hastalarda ise %50,9'unda remisyon, %47,2 hastada tedaviye eksik yanıt ve %1,9 hastada tedavi başarısızlığı görüldü, %48,1 hastada nüks vardı. Tüm OİH tanılı hastalarda AAR değerinin 1'in üzerinde olması, karaciğer biyopsisinde zon-3 tutulumu, PNL çokluğu, eozinofil varlığı ve konfluent nekroz varlığı tedaviye kötü yanıt ile ilişkili saptandı. İzole OİH tanılı hastalarda ise FİB-4 indeksinin yüksek olması, karaciğer biyopsisinde zon-3 tutulumu, PNL çokluğu ve konfluent nekroz varlığı tedaviye kötü yanıt ile ilişkiliydi. Sonuç: OİH, etyolojisi net olmayan kronik ve ilerleyici inflamatuar bir karaciğer hastalığıdır. Tedavi edilmeyen OİH, fibrozisin siroza ilerlemesine ve son dönem karaciğer hastalığına yol açar. Tedaviye yanıtı etkileyen faktörler, tedavi öncesinde hastalığın prognozu hakkında bilgi verir ve bu doğrultuda tedavi planlamamıza imkan sağlar. Çalışmamızda tedaviye kötü yanıt ile ilişkili parametreler, AAR değerinin 1'in üzerinde olması, FİB-4 indeksinin yüksek olması, karaciğer histolojisinde, zon-3 tutulumu, PNL çokluğu, eozinofil varlığı ve konfluent nekroz varlığıdır. Literatürde bu parametrelerin tedaviye yanıt ile ilişkisi çalışılmamıştır. Bulgularımız literatüre ışık tutacak niteliktedir, ancak bu ilişkinin daha iyi anlaşılması için prospektif çalışmalara ihtiyaç vardır. Introduction: The aim of our study is to investigate the characteristics of AIH patients followed in the Department of Gastroenterology at Süleyman Demirel University and the noninvasive and invasive factors that determine the response to treatment. Materials and Methods: For our study, the data of 71 patients with AIH who were followed up in Süleyman Demirel University Research and Practice Hospital, Department of Gastroenterology were retrospectively analyzed. Median age at diagnosis was 48 years. 32.4% of the patients had extrahepatic autoimmune disease, The most common autoimmune thyroid diseases were seen. Overlap syndrome was seen in 25.3% of the patients. AIH-PCK overlap (23.9%) was detected more frequently than AIH-PCK overlap (1.4%). The most common complaints were jaundice (21.1%), weakness-fatigue (15.5%) and abdominal pain (11.3%). 81.7% of the patients were evaluated as type 1 AIH, 1.4% as type 2 AIH, and 15.5% as autoantibody negative AIH. 42.3% of the patients had cirrhosis at the time of diagnosis. The most common ultrasonographic finding was chronic liver disease findings (43.7%) and hepatosteatosis (31%). Combination therapy containing steroids and azathioprine was preferred more than monotherapy (2.8% versus 95.8%). Remission was observed in 45.1% of the patients, incomplete response to treatment in 53.5%, and treatment failure in 1.4%, recurrence was seen in 46,9% of patients. In patients with isolated AIH, 50.9% had remission, 47.2% had an incomplete response to treatment, and 1.9% had treatment failure, there was recurrence in 48.1% of the patients. AAR value above 1, zone-3 involvement in liver biopsy, abundance of PNL, presence of eosinophils and confluent necrosis were found to be associated with poor response to treatment in all AIH patients. High FIB-4 index, zone-3 involvement in liver biopsy, multiple PNL and confluent necrosis were associated with poor response to treatment in patients with isolated AIH. Conslusion: AIH is a chronic and progressive inflammatory liver disease of unclear etiology. Untreated AIH leads to progression of fibrosis to cirrhosis and end-stage liver disease. Factors affecting response to treatment provide information about the prognosis of the disease before treatment. and allows us to plan treatment accordingly. The parameters associated with poor response to treatment in our study were AAR value above 1, high FIB-4 index, zone-3 involvement in liver histology, PNL abundance, eosinophil presence, and presence of confluent necrosis. The relationship between these parameters and response to treatment has not been studied in the literature. Our findings shed light on the literature, but prospective studies are needed to better understand this relationship.
