Amaç: KLL batı toplumlarında en sık görülen ve prognozu oldukça değişken bir hastalıktır. Hastaların hangi risk grubunda ve hangi prognostik faktörlere sahip olduğunu belirlemek, tahmini yaşam süresi ve uygulanacak tedaviye verilecek yanıt hakkında fikir vermektedir. Bu amaçla Rai ve Binet gibi evrelendirme modelleri ile yaş, cinsiyet, LDT, mutlak lenfosit sayısı, serum beta-2 mikroglobulin gibi laboratuvar tetkiklerine dayalı belirteçler oluşturulmuştur. Bu çalışmada, kullanılan prognostik faktörlere ek olarak beden kitle indeksinin hastalığın seyrinde önemli bir rolü olup olmadığı değerlendirilmiştir. Materyal ve Metod: Araştırmamızda Süleyman Demirel Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı'na 2006-2020 yılları arasında KLL tanısı ile takip edilen ve bu tanı ile kemoterapi alan 58 hasta dahil edildi. Hastaların yaş, cinsiyet, evre, kemoterapi protokolü, kaç kür kemoterapi aldığı kaydedilip, hastaların tedaviye yanıtları değerlendirildi. Aynı zamanda hastaların kemoterapiye başlangıç zamanındaki HB, WBC, PLT düzeyi, LDH düzeyleri, LDT, FISH ile mutasyon analizleri, sağkalım, progresyonsuz sağkalım süresi, mortalite durumu kaydedildi ve bu parametrelerin BKİ ile ilişkisi değerlendirildi. Veriler, IBM SPSS.23 programına aktarılarak istatistiksel analizlerle değerlendirildi. Bulgular: Çalışmamıza dahil edilen 58 hastanın 16'sı (%27,6) kadın, 42'si (%72,4) erkekti. Cinsiyet ile BKİ grupları arasında istatistiksel olarak anlamlı bir ilişki saptanmadı (p>0,05). Hastaların mortalite durumu ile BKİ düzeyleri arasındaki ilişki değerlendirildiğinde, BKİ arttıkça mortalite durumunun arttığı tespit edildi (p=0,030). Çalışmamıza dahil edilen hastaların BKİ grupları ile medyan sağkalım ayı arasında istatistiksel olarak anlamlı bir ilişki gözlenmedi (p>0,05). Normal kilolu olan hastaların 11'inde (%34,4), kilolu olanların 5'inde (%26,3) relaps gözlendi. Relaps durumu ile BKİ grupları arasında istatistiksel olarak anlamlı bir ilişkiye rastlanmadı (p>0,05). Sonuç: KLL tanılı obez hastalarda mortalitenin anlamlı derecede artmış olduğu tespit edildi. İstatistiksel açıdan anlamlı olmasa da kilolu hastalarda progresyonsuz sağ kalımın ve toplam sağ kalımın normal kilolu olan hastalara göre daha fazla olduğunu saptadık. Konu ile ilgili, prospektif ve hasta sayısının daha fazla olduğu çalışmaların yapılması gerekmektedir. Aim: CLL is the most common disease in western societies and its prognosis is highly variable. Determining the risk group and prognostic factors of the patients gives an idea about the estimated life expectancy and the response to the treatment to be applied. For this purpose, staging models such as Rai and Binet and markers based on age, gender, LDT, absolute lymphocyte count, and serum beta-2 microglobulin were created. In this study, in addition to the prognostic factors used, it was evaluated whether BMI played an important role in the course of the disease. Material and Method: In our study, 58 patients who were followed up with the diagnosis of CLL in the Department of Hematology, Faculty of Medicine, Süleyman Demirel University between 2006-2020 and received chemotherapy with this diagnosis were included. The patients' age, gender, stage, chemotherapy protocol, how many cycles of chemotherapy were recorded, and the response of the patients to the treatment was evaluated. Also, at the time of starting chemotherapy HB, WBC, PLT level, LDH levels were recorded, LDT, mutation analyzes with FISH, survival, progression-free survival time, mortality status, and the relationship of these parameters with BMI was evaluated. Data were transferred to IBM SPSS.23 program and evaluated with statistical analysis. Results: Of the 58 patients included in our study, 16 (27.6%) were female and 42 (72.4%) were male. No statistically significant relationship was found between gender and BMI groups(p>0,05). When the relationship between the mortality status of the patients and their BMI levels was evaluated, it was determined that the mortality status increased as the BMI increased (p=0.030). No statistically significant correlation was observed between the BMI groups and the median survival month of the patients included in our study (p>0.05). Relapse was observed in 11 (34.4%) patients with normal weight and 5 (26.3%) patients with overweight. No statistically significant relationship was found between relapse status and BMI groups (p>0.05). Conclusion: It was determined that mortality was significantly increased in patients with obesity diagnosed with CLL. Although not statistically significant, we found that progression-free survival and overall survival were higher in overweight patients than in patients with normal weight.
