Over kanseri en ölümcül jinekolojik malignitedir ve dünya çapında kadınlarda kansere bağlı ölümlerin beşinci önde gelen nedeni olarak bilinir. Çoğu hastada kanser, karın içi metastazlar ile ileri evrede teşhis edilmektedir. Epitelyal Over kanseri hastalarında 5 yıllık genel sağkalım (OS) oranı %49'a ulaşmaktadır. Günümüzde postoperatif rezidüel tümör, ileri evre over kanseri hastalarında açık ara farkla en önemli prognostik faktör olarak değerlendirilmektedir. Over kanserinde yapılan ameliyatının amacının mümkün olduğu kadar büyük tümörü çıkarmak olduğu görüşü kabul görmüştür. Epitelyal over kanserinde karşılaşılan en önemli meydan okumalardan birisi barsak tutulumunun olmasıdır. Optimal sitoredüksiyona ulaşabilmek için barsak rezeksiyonu yapılması gerekmektedir. Bu çalışmanın amacı barsak rezeksiyonlarının over kanserinin seyri üzerindeki etkisini kısa dönemde ve uzun dönemde değerlendirmektir. Ayrıca barsak rezeksiyonu yapılan ve yapılmayan hasta gruplarında sekonder sekonder sitoredüksiyon ihtiyacını analiz ettik. Preoperatif, intraoperatif, postoperatif sonuçlarına bağlı kategorik değişkenler arasındaki ilişkiler, uygunluğuna göre ki-kare test veya fisher 's exact test kullanılarak analiz edildi. Sürekli değişkenler ise mann whitney-u testi ile değerlendirildi. Sağkalım hesaplamasında log-rank test kullanıldı. Genel sağkalımın prognostiği olduğu varsayılan faktörlerin bağımsız önemini belirlemek için cox orantılı tehlikeler modeli kullanıldı. Çalışma süreci boyunca; evre II-IVB olan 127 hastaya sitoredüksiyon yapıldı. 58 hastaya barsak rezeksiyonu uygulandı. Operasyonların 6 tanesi hariç hepsi, jinekolojik onkoloji ekibi tarafından yapıldı. Barsak rezeksiyonlarının ise 2 tanesi dışında hepsi jinekolojik onkoloji ekibi tarafından gerçekleştirildi. Biz yaptığımız bu retrospektif çalışmada; sitoredüksiyon sırasında barsak rezeksiyonu yapılmasının, genel sağkalımda olumsuz bir etki yapmadığını ve ilk 90 gündeki mortalite ve genel mortalite üzerine artışa sebep olmadığını ve hastanede daha uzun süreli yatışın ana nedeni olmadığını gözlemledik. Ayrıca önemli bulgularımızdan birisi de; primer sitoredüksiyon sırasında barsak rezeksiyonu yapılanlar ve yapılmayanlar karşılaştırıldığında; tedavi takip sürecinde sekonder sitoredüksiyona gereksinim duyma olasılığının daha düşük olduğunu saptadık. İnterval debulking cerrahisi ve suboptimal rezeksiyonunun sağkalımı olumsuz etkilediğini, optimal sitoredüksiyon ve lenf nodu diseksiyonunun sağkalımı olumlu etkilediğini bulduk. Barsak rezeksiyonu yapılan hastalarda kan ürünü transfüzyon oranı, yoğun bakım yatışı, ilk üç aydaki hastane yatış sayısı, değerlerinin artmış olduğu bulundu. Sonuç olarak; barsak tutulumu olan over kanseri hastalarında rezeksiyonun yapılabilmesi, rezidü tümör bırakmadan operasyonun tamamlanabilmesi cerrahi başarı ve bunun neticesinde sağkalımdaki artış açısından önemlidir. Ovarian cancer is the mortal gynecological malignancy and is recognized as the fifth leading cause of cancer-related death in women worldwide. In most patients, cancer is diagnosed at an advanced stage with intra-abdominal metastases. The 5-year overall survival (OS) rate reaches 49% in patients with epithelial ovarian cancer. Today, postoperative residual tumor is considered by far the most important prognostic factor in patients with advanced ovarian cancer. It is accepted that the aim of surgery in ovarian cancer is to remove the tumor as large as possible. One of the most important challenges in epithelial ovarian cancer is bowel involvement. Bowel resection is required to achieve optimal cytoreduction. The aim of this study is to evaluate the short-term and long-term effects of bowel resections on the course of ovarian cancer. We also analyzed the need for secondary cytoreduction in patient groups with and without bowel resection. Relationships between categorical variables related to preoperative, intraoperative, and postoperative outcomes were analyzed using either the chi-square test or the Fisher's exact test, as appropriate. Continuous variables were evaluated with the Mann Whitney-U test. Log-rank test was used for survival calculation. A cox proportional hazards model was used to determine the independent significance of factors presumed to be prognostic for overall survival. During the study process; cytoreduction was performed in 127 patients with stage II-IVB. Bowel resection was performed in 58 patients. All but 6 of the operations were performed by the gynecological oncology team. All but 2 bowel resections were performed by the gynecological oncology team. In this retrospective study we conducted; We observed that performing bowel resection during cytoreduction did not have a negative effect on overall survival and did not cause an increase in mortality and overall mortality in the first 90 days, and was not the main cause of longer hospital stay. In addition, one of our important findings is; when bowel resection was performed during primary cytoreduction and those who did not; We found that the probability of needing secondary cytoreduction during the treatment follow-up period is lower. We found that interval debulking surgery and suboptimal resection negatively affected survival, while optimal cytoreduction and lymph node dissection affected survival positively. It was found that blood product transfusion rate, intensive care hospitalization, number of hospitalizations in the first three months, values increased in patients who underwent bowel resection. In conclusion; Performing resection in patients with ovarian cancer with bowel involvement and completing the operation without leaving any residual tumor are important for surgical success and the resulting increase in survival.
