Prostat adenokarsinomlu hastalarda, tedavi öncesi yapılan Ga-68 PSMA PET/BT görüntülerinden elde edilen lezyon bazlı volümetrik ve tekstür parametreleri ile tedavi yanıtı arasındaki ilişkiyi incelemeyi amaçladık. Aralık 2018-Kasım 2022 tarihleri arasında prostat kanseri tanısıyla hem tedavi öncesinde hem de tedavi sonrasında Ga-68 PSMA PET/BT görüntülemesi yapılan 226 hasta çalışmaya dahil edildi. Toplamda 452 Ga-68 PSMA PET/BT görüntüsü retrospektif ve karşılaştırılmalı şekilde incelendi. Tedavi yanıt değerlendirmesine göre miks yanıt veren 97 hastanın bazal ve tedavi sonrasındaki kan parametreleri kaydedildi. Lezyonların PSMA ekspresyon durumuna göre görüntüler tekrar değerlendirildi. Lezyon bazında miks yanıt veren (kemik, lenf nodu) 82 hastanın 1012'si kemik, 136'sı lenf nodu olmak üzere 1148 metastatik lezyonu PSMA ekspresyonu artış ve azalışına göre iki gruba ayrıldı. Görüntüler LIFEx programında değerlendirildi. Metastatik kemik lezyonları ve lenf nodları %40 threshold ile çizildi, volümetrik ve tekstür özellikleri elde edildi. Bu veriler ile PSMA ekspresyonu değişimi arasındaki ilişki incelendi. Kemik lezyonlarında PSMA ekspresyonu azalan lezyonlarda SUVmax, SUVmean, SUVmin değerleri daha düşüktü (referans değerler sırasıyla: 6,51, 4,71 ve 2,49). PSMA-TV ise daha yüksekti (referans değer: 8,92 cm3). TL-PSMA değerinde anlamlı bir farklılık izlenmedi. Lojistik regresyon analizinde ise TL-PSMA -0.007 kat; PSMA-TV ise 0,056 kat PSMA ekspresyonu değişimini etkilemekteydi. Lenf nodlarında, PSMA ekspresyonu azalan lezyonlarda SUVmax ve SUVmin değerleri daha düşüktü (referans değerler sırasıyla: 5,40 ve 3,58). SUVmean, PSMA-TV ve TL-PSMA değerlerinde anlamlı bir farklılık izlenmedi. Lojistik regresyon analizinde ise SUVmax -0.953 kat; SUVmean ise 1,74 kat PSMA ekspresyonu değişimini etkilemekteydi. Tekstür parametrelerine bakıldığında, lezyon bazında daha çok heterojeniteyi gösteren; intensity-based özelliklerden kurtosis, GLCM-Entropy, GLCM-Contrast, GLCM-Dissimilarity ve GLSZM-ZS.ENTR gibi parametreler PSMA ekspresyonu artan lezyonlarda daha yüksek iken; homojeniteyi gösteren intesity histogram-uniformity, GLCM-Angular second moment, GLCM-Inverse difference, GLCM-Inverse difference moment ve GLSZM-LZHGE gibi parametreler daha düşüktü. Bu çalışma ile Ga-68 PSMA PET görüntülerinden elde edilen tekstür parametreleri lezyon bazlı değerlendirildiğinde, lezyonların heterojenitesini göstererek tedavi yanıtını öngörmede rolü olabileceğini gösterdik. Baseline and post-treatment blood parameters of 97 patients with mixed response according to treatment response evaluation were recorded. Images were re-evaluated according to the PSMA expression status of the lesions. In 82 patients with mixed response (bone, lymph node), 1148 metastatic lesions (1012 bone and 136 lymph node) were divided into two groups according to the increase and decrease of PSMA expression. Images were evaluated in the LIFEx program. Metastatic bone lesions and lymph nodes were drawn with a 40% threshold and volumetric and textural features were obtained. The relationship between these data and the change in PSMA expression was analyzed. In bone lesions, SUVmax, SUVmean and SUVmin values were lower in lesions with decreased PSMA expression (reference values: 6.51, 4.71 and 2.49, respectively). PSMA-TV was higher (reference value: 8.92 cm3). No significant difference was observed in the TL-PSMA value. In logistic regression analysis, TL-PSMA and PSMA-TV affected the change in PSMA expression by -0.007 and 0.056 folds, respectively. In lymph nodes, SUVmax and SUVmin values were lower in lesions with decreased PSMA expression (reference values: 5.40 and 3.58, respectively). No significant difference was observed in SUVmean, PSMA-TV and TL-PSMA values. In logistic regression analysis, SUVmax was -0.953-fold and SUVmean was 1.74-fold influential on the change in PSMA expression. When texture parameters are analyzed, intensity-based parameters such as kurtosis, GLCM-Entropy, GLCM-Contrast, GLCM-Dissimilarity and GLSZM-ZS. ENTR were higher in lesions with increased PSMA expression, while parameters such as intensity histogram-uniformity, GLCM-Angular second moment, GLCM-Inverse difference, GLCM-Inverse difference moment and GLSZM-LZHGE were lower. In this study, we demonstrated that when texture parameters obtained from Ga-68 PSMA PET images are evaluated on a lesion basis, they may play a role in predicting treatment response by showing the heterogeneity of lesions.
