Amaç: İmmüntrombositopeni çocuk ve yetişkinler de görülen antikor aracılı trombositlerin splenik makrofajlar tarafından hızlı destrüksiyonu veya kemik iliğin de megakaryositer seriden yetersiz trombosit üretimi ile karakterize bir hastalıktır. Yaşla birlikte sıklığı artmakla birlikte kadınlar da daha sık görülmektedir. Relaps ve remisyonlarla seyreden bir hastalıktır. Hastaların çoğunluğu birinci basamak tedavi ile remisyona girmekle birlikte takiplerin de relaps olan ve ikinci basamak tedavi ihtiyacı olan hastalar da mevcuttur. Trombosit parametleri günümüz de giderek ITP prognozu açısından önem kazanmaktadır. Bu çalışma trombosit parametlerinden MPV ve PCT'nin ITP hastalarında prognostik önemini ortaya koymak amacı ile yapılmıştır. Materyal ve Metot: Süleyman Demirel Üniversitesi Hematoloji Bilim Dalı'nda 2005 ile 2022 yılları arasında İmmün Trombositopenik Purpura tanısı alan hastalar retrospektif olarak taranmıştır. Hastaların yaş, cinsiyet, aldığı tüm tedaviler, hemogram parametreleri ve ek hastalıkları kayıt edilmiştir. Hastaların MPV, PLT, PCT değerleri aldığı tedavi öncesi ve sonrası olarak gruplandırıldı. Bu parametreler daha sonra verilerin analizi SPSS 17.0 for Windows programı ile yapıldı. Bulgular: Çalışma, immüntrombositopeni tanısı ile takipli tedavi almış toplam 123 hematoloji hastası ile tamamlandı. Hastaların %66,7 'si kadın ve yaş ortalaması 56,79±18,37 yıl (medyan 58) olarak belirlendi. Hastaların tamamı (%100) birinci basamak tedaviyi almıştı. Hastaların %69,9'u birinci basamak tedavi sonrası remisyona girmişti. Hastaların %30,1'i birinci basamak tedaviye cevapsız olup ikinci basamak tedavi aldı. Birinci basamak tedavi sonrası remisyona giren hastaların %27,6'sında ise relaps gözlendi ve ikinci basamak tedavi verildi. Birinci basamak tedavi ile remisyon giren hastaların tedavi öncesi ve sonrası MPV değeri anlamlı oran da düşerken PLT ve PCT değerlerinde anlamlı oranda artış oldu. Birinci basamak tedavi sonrası relaps olan ve ikinci basamak tedavi alan hastaların tedavi sonrası MPV düzeyi anlamlı oranda düşerken, PCT ve PLT düzeylerin de anlamlı ölçüde artış saptandı. Birinci ve ikinci basamak tedavi alan hastaların PCT düzeylerinde sayısal olarak fark olmasına rağmen istatiksel olarak tedavi öncesi ve sonrası anlamlı fark saptanmadı. Relaps olan hasta grubun da alınan tedavi sayısı ile tedavi öncesi PCT değeri arasın da anlamlı fark saptanmadı. Sonuç: ITP hastaların da trombosit parametrelerinden MPV tedavi ile düşerken PCT ve PLT artış göstermektedir. Ancak hastaların prognozu ve aldığı toplam tedavi sayısı açısından PCT'nin prognostik önemi çalışmamızda saptanmamıştır. Çalışmamız bu açıdan literatür ile uyumsuz saptanmıştır. Çalışmamız ile literatür arasındaki bu uyumsuzluğun nedenleri arasında, retrospektif bir çalışma olması, sekonder ITP hastalarını dışlamamamız ve dolayısıyla PCT'nin bu sebeple etkilenmiş olması aklımıza gelmektedir. Purpose: Immune thrombocytopenia is a disease seen in children and adults, characterized by rapid destruction of antibody-mediated platelets by splenic macrophages or insufficient platelet production from the megakaryocytic series in the bone marrow. Although its frequency increases with age, it is also more common in women. It is a disease that progresses with relapses and remissions. Although the majority of patients go into remission with first-line treatment, there are also patients who relapse during follow-up and need second-line treatment. Nowadays, platelet parameters are becoming increasingly important in terms of ITP prognosis. This study was conducted to reveal the prognostic importance of platelet parameters MPV and PCT in ITP patients. Material and Method: Patients diagnosed with Immune Thrombocytopenic Purpura between 2005 and 2022 at Süleyman Demirel University Hematology Department were retrospectively screened. The patients' age, gender, all treatments received, hemogram parameters and additional diseases were recorded. The patients' MPV, PLT, PCT values were grouped as before and after treatment. These parameters were then analyzed with the SPSS 17.0 for Windows program. Results: The study was completed with a total of 123 hematology patients who received follow-up treatment with a diagnosis of immunothrombocytopenia. 66.7% of the patients were female and the average age was 56.79±18.37 years (median 58). All patients (100%) had received first-line treatment. 69.9% of the patients went into remission after first-line treatment. 30.1% of the patients did not respond to first-line treatment and received second-line treatment. Relapse was observed in 27.6% of the patients who went into remission after first-line treatment and second-line treatment was given. While the MPV values of patients who achieved remission with first-line treatment decreased significantly before and after treatment, there was a significant increase in PLT and PCT values. While the post-treatment MPV levels of patients who relapsed after first-line treatment and received second-line treatment decreased significantly, PCT and PLT levels were also significantly increased. Although there was a numerical difference in the PCT levels of patients receiving first and second-line treatment, no statistically significant difference was detected before and after treatment. In the relapsed patient group, no significant difference was detected between the number of treatments received and the pre-treatment PCT value. Conclusion: In ITP patients, while MPV, one of the platelet parameters, decreases with treatment, PCT and PLT increase. However, the prognostic significance of PCT in terms of the prognosis of the patients and the total number of treatments they received was not determined in our study. Our study was found to be incompatible with the literature in this respect. Among the reasons for this inconsistency between our study and the literature, we think that it was a retrospective study, we did not exclude secondary ITP patients, and therefore PCT was affected for this reason.
