Over kanseri jinekolojik kanserler arasında ölüm oranı en yüksek olanıdır. Over kanserinin tedavisinde sitoredüktif cerrahi ve kemoterapi önemli rol oynar. Cerrahinin amacı tam sitoredüksiyona ulaşmak ve tüm makroskopik hastalıkları ortadan kaldırmaktır. Tam sitoredüksiyona ulaşılan başalarılı cerrahi, adjuvan kemoterapiden maksimumu verim almaya ve sağkalımda artışı sağlar. Prognoz cerrahi başarıya göre şekillenmektedir. Bundan dolayı preoperatif hasta seçimi son derece önemlidir. Hasta seçiminde, hastanın performans skoru, tümörün yaygınlığını gibi faktörler iyi değerlendirilmelidir. Artan yaşla beraber over kanserinin insidansı, hastaların komorbiditide durumu ve hastalığın evresinde artış görülür. Yaş over kanserinin prognozunda bağımsız faktör olarak yerini almaktadır. Literatürde ileri yaşlı hastalar için cerrahi kriterler net olarak belirlenememiştir. Bu çalışma ile sitoredüktif cerrahi sonrasında yaşın sağkalım üzerine etkilerini araştırmak ve literatürdeki anlam karışıklığını gidermeyi amaçlandırılmıştır. 01/09/2007 ve 31/12/2022 tarihleri arasında Süleyman Demirel Üniversitesi Hastanesi'nde epitelyal over kanseri nedeni ile opere edilen hastaların tıbbi kayıtları retrospektif olarak değerlendirilmiştir. Evre IIIA-IVB arası tanısı alan 138 hasta çalışmaya dahil edilmiştir. Retrospektif olarak hasta bilgileri hastane arşivinden ve hastane bilgi sisteminden karşılanmıştır. Çalışmaya dahil edilen hastalar 65, 70 ve 75 yaşlarına göre gruplandırarak incelendi. 75 yaş ve üzeri toplam 9 hasta olduğundan bu yaş grubu ayrı ayrı incelendi. Hastaları yaşa göre sağkalım sürelerini incelendi. Ayrıca hastalığın evresi , histopatolojik türü, performans skoru, operasyon sırasında yapılan işlemler, postoperatif yoğun bakım ihtiyacı ve hastaneye tekrar yatış sayısı gibi diğer parametreleri de incelendi. 75 yaşa kadar ileri evre over kanseri tanılı hastalar radikal cerrahiyi tolere edebilirlerse postoperatif süreçleri ve sağkalım sonuçları istatistiksel olarak genç hastalarla benzer sonuçlanmıştır. Cerrahi amaç mutlaka tam sitoredüksiyon veya optimum sitoredüksiyon olmalıdır. Hastanın performans skoru iyi ise, 75 yaşına kadar ameliyatın güvenle yapılabileceği kanaatindeyiz. Ovarian cancer has the highest mortality rate among gynecological cancers. İn the treatment of ovarian cancer cytoreductive surgery and chemotherapy play a significant role. The aim of surgery is to reach complete cytoreduction and eliminate all macroscopic disease. A successful surgery that achieves complete cytoreduction takes maximum benefit from adjuvant chemotherapy and increases survival. Prognosis depends on surgical success. Therefore, preoperative patient selection is extremely important. In patient selection, factors such as the patient's performance score and the extent of the tumor should be evaluated carefully. The incidence of ovarian cancer, patients' comorbidities and the stage of the disease increases with age. Age is an independent factor in the prognosis of ovarian cancer. Surgical criteria for elderly patients have not been clearly determined in the literature. This study aimed to investigate the effects of age on survival after cytoreductive surgery and to eliminate the confusion in the literature. Medical records of patients operated on for epithelial ovarian cancer at Süleyman Demirel University Hospital between 01/09/2007 and 31/12/2022 were evaluated retrospectively. 138 patients diagnosed with stages from IIIA to IVB were included in the study. Personal health information was obtained from the hospital archive and hospital information system retrospectively. The patients included in the study were examined by grouping them according to age 65, 70 and 75. There are a total of 9 patients aged 75 and over so this age group was examined separately. The survival times of the patients were examined according to their age. In addition, other parameters such as the stage of the disease, histopathological type, performance score, procedures performed during the operation, postoperative intensive care need and number of hospital readmissions were also examined. If patients diagnosed with advanced ovarian cancer up to the age of 75 can tolerate radical surgery, their postoperative processes and survival outcomes are statistically similar to younger patients. The surgical aim must be complete cytoreduction or optimum cytoreduction. We conclude that if the patient's performance score is good, surgery can be performed safely until the age of 75. Further more we suggest that it would be beneficial to conduct a study with a larger sample size over 75 years of age. Keywords : Ovarian Cancer, Cytoreductive Surgery, Elders, Survival.
