Karbonmonoksit, içinde karbon ihtiva eden maddelerin tam olmayan yanmaları sonucu ortaya çıkan renksiz, kokusuz, irritan olmayan ve toksik bir gazdır. Zehirlenme bu gazın inhalasyonu yolu ile vücuda alınması ile başlar. Tüm dünyada zehirlenmelere bağlı ölümler içerisinde önemli bir yer tutar. Karbonmonoksit zehirlenmesinin tanısını koymak çok kolay olsada klinik bulguları spesifik olmadığı için tanısı sık atlanmaktadır. Tanı invaziv olarak kan gazı ile veya non-invaziv olarak co-oksimetri cihazları ile konur. Acil serviste tanının erken konulması tedavinin erken başlanması mortalite ve morbidite azaltır. Bu çalışmada acil servise karbonmonoksit zehirlenmesi şüphesi ile başvuran ve tanı alan hastaların hemogram, biyokimya, kan gazı değerlerini kaydettik. Hastaların PLT, MPV, PCT ve COHb değerlerinin mortalite, prognoz ve tedavi şeklinin belirlenmesine etkisini araştırdık. Süleyman Demirel Üniversitesi Tıp Fakültesi Acil Servisine 1 Ocak 2015- 1 Ocak 2022 tarihleri arasında başvurmuş, 18 yaş üstü herhangi bir hematolojik hastalığı olmayan ve tetkiklerine tam ulaşılan 127 hasta dahil edilmiştir. Çalışmaya alınan hastaların 74'kadın 53'ü erkek ve yaş ortalaması 50±17 bulunmuştur. Hastaların en çok kış mevsiminde acil servise başvurduğu, en çok başvurunun ise Ocak ayında olduğu görülmüştür. Hastaların COHb düzeyi ortalaması 18,64±11,7 olarak bulundu. Sigara içen ve içmeyenler arasında trombosit indeksleri ve COHb düzeyleri arasında anlamlı bir fark bulunmadı. Çalışmamızda PLT, MPV ve PCT'nin COHb düzeyi ile bir korelasyon göstermediği bulunmuş olup, hastaların mortalitesini tahmin etmede de PCT ve PLT spesifitesi düşük olsa da sensitif bulunmuştur. PLT, MPV ve PCT'nin yatış-taburculuk, mortalite ve HBOT ihtiyacını belirlemede korelasyon göstermediği görülmüş olup karar vermede iyi bir parametre olmadıkları bulunmuştur. Yine PLT, MPV ve PCT'nin HFNO kullanımı ile bir korelasyonu bulunamamış olup HFNO kullanımı ile sadece COHb düzeyi korele çıkmıştır. Sonuç olarak trombosit indeksleri hemogram içerisinde rutin olarak bakıldığı, akut inflamatuvar süreçlerden etkilendiği ve ucuz olduğu için CO zehirlenmesinde klinisyene yardımcı olabilir mi sorusuna yanıt aradık. Mortalite ve prognoz belirlemek için değerlendirdiğimiz PLT, MPV, PCT ve COHb düzeylerinin mortaliteyi öngörmede yeterli olmadığı bulunmuş, hastanın kliniği, komorbiditeleri ve CO maruziyet süreleri gibi sebeplerinde mortalite ve prognoz üzerinde etkilerinin olduğu görülmüştür. Carbon monoxide is a colorless, odorless, non-irritant and toxic gas that results from the incomplete combustion of substances containing carbon. Poisoning begins with the inhalation of this gas into the body. It occupies an important place among deaths due to poisoning worldwide. Although it is very easy to diagnose carbon monoxide poisoning, the diagnosis is often missed because the clinical findings are not specific. The diagnosis is made invasively by blood gas or non-invasively by co-oximeter devices. Early diagnosis in the emergency department reduces mortality and morbidity by starting treatment early. In this study, we recorded the hemogram, biochemistry, blood gas values of patients who presented to the emergency department with suspicion of carbon monoxide poisoning and were diagnosed. We investigated the effect of PLT, MPV, PCT and COHb values on mortality, prognosis and treatment modality of patients. 127 patients who applied to Süleyman Demirel University Faculty of Medicine Emergency Department between January 1, 2015 and January 1, 2022, who were over 18 years old and had no hematological disease and had full access to their tests were included in the study. Of the patients included in the study, 74 were female and 53 were male and the mean age was 50±17. It was seen that most of the patients applied to the emergency department in winter and most of the applications were in January. The mean COHb level of the patients was found to be 18.64±11.7. There was no significant difference between smokers and non-smokers in terms of platelet indices and COHb levels. In our study, it was found that PLT, MPV and PCT did not show a correlation with COHb level, and PCT and PLT were found to be sensitive although their specificity was low in predicting mortality of patients. It was seen that PLT, MPV and PCT did not show a correlation in determining hospitalization-discharge, mortality and HBOT need, and they were not good parameters in decision making. Again, no correlation was found between PLT, MPV and PCT and HFNO use, and only COHb level correlated with HFNO use. As a result, we searched for an answer to the question whether platelet indices can help clinicians in CO poisoning as they are routinely looked at in hemogram, affected by acute inflammatory processes and cheap. It was found that PLT, MPV, PCT and COHb levels were not sufficient to predict mortality and prognosis for determining mortality and prognosis. It was seen that reasons such as patient's clinic, comorbidities and CO exposure time also had effects on mortality and prognosis.
