Santral venöz sinüslerde (SVS), trombüsle karışabilen birçok varyasyon ve artefakt görülebilmektedir. Bu nedenle SVS anatomisi ve varyasyonları radyolojik raporlamada oldukça önemlidir. Biz de bu çalışmada SVS varyasyonlarını tespit etmenin önemini ortaya koymakla birlikte "SVS trombozu" şüphesi olan hastalarda da manyetik rezonans venografi (MRV) çekim protokollerini hasta özelinde optimize etmeyi amaçladık. Çalışmamıza Ocak 2015-Aralık 2018 tarihleri arasında 3-dimensional magnetization prepared-rapid gradient echo (3D MP-RAGE) ile 2-dimensional time-of-flight (2D TOF) MRV çekimleri yapılmış 58'i kadın, 92'si erkek toplam 150 hasta dahil edildi. Her hastanın SVS aplazi, hipoplazi ve diğer varyasyonlarını sınıflandırdık. 3D MP-RAGE MRV'de SVS'lerin çapları ile 2D TOF MRV'de flow gap varlığını inceledik. Sol TS asimetri derecesi grade-3 (aplazi) olma olasılığı, kadınlarda anlamlı düzeyde yüksek saptandı. 3D MP-RAGE MRV'de yaptığımız sinüs çap ölçümlerinde erkeklerde hem sağ hem de sol SS çap ortalaması ile posterior SSS çap ortalamasının, kadınlara göre istatistiksel olarak anlamlı derecede yüksek olduğu saptandı. 2D TOF MRV'de TS'lerdeki flow gap varlığı ile konfluens sinuum tipleri arasında istatistiksel olarak anlamlı bir ilişki olduğu görüldü. 3D MP-RAGE MRV'deki TS'lerin çap ortalaması ile 2D TOF MRV'deki flow gap ilişkisi değerlendirildiğinde TS'lerin çapları küçüldükçe, flow gap görülme olasılığının arttığı saptandı. 3D MP-RAGE MRV'de değerlendirilen SS'lerin de çap ortalamasının düşmesiyle aynı taraf TS'lerde flow gap görülme olasılığının arttığı tespit edildi. 3D MP-RAGE MRV'de değerlendirilen TS'lerin asimetri derecesi ile 2D TOF MRV'de değerlendirilen flow gap düzeyleri arasında istatistiksel olarak anlamlı bir ilişki olduğu elde edildi. Bu çalışmayla elde edilen sonuçlar, SVS varyasyon-trombüs ayrımı noktasında her ne kadar kontrastlı T1 ağırlıklı 3D MPRAGE MRV'nin 2D TOF MRV'ye üstün olduğunu gösterse de bu iki tetkikin kombinasyonunun daha güvenilir sonuçlar vereceğini düşünmekteyiz. MRV'nin yanısıra hızlı, kolay ulaşılabilir ve görece ucuz bir tetkik olan Doppler ultrasonografinin de SVS varyasyon-trombüs ayrımında yararlı olacağını öngörmekteyiz. Bu konuda daha kapsamlı çalışmalara ihtiyaç vardır. Numerous variations and artifacts that can be mistaken for thrombosis can be observed in the central venous sinuses (CVS). Therefore, the anatomy and variations of the CVS are highly significant in radiological reporting. In this study, we aimed to highlight the importance of identifying CVS variations and to optimize patient-specific magnetic resonance venography (MRV) imaging protocols for patients with suspected "CVS thrombosis". Between January 2015 and December 2018, 150 patients (58 females, 92 males) were included in our study, in which three-dimensional magnetization-prepared rapid gradient echo (3D MP-RAGE) and two-dimensional time-of-flight (2D TOF) MRV images were obtained. We classified the CVS aplasia, hypoplasia, and other variations in each patient. We examined the diameters of CVS in 3D MP-RAGE MRV and the presence of flow gaps in 2D TOF MRV. The probability of grade-3 (aplasia) left transverse sinus (TS) asymmetry was found to be significantly higher in females. In 3D MP-RAGE MRV, our sinus diameter measurements showed that both the mean diameter of the right and left sigmoid sinuses and the mean diameter of the posterior superior sagittal sinus (SSS) in males were statistically significantly higher compared to females. A statistically significant relationship was observed between the presence of flow gaps in TSs in 2D TOF MRV and confluence sinuum types. When evaluating the mean diameter of TSs in 3D MP-RAGE MRV and the flow gap relationship in 2D TOF MRV, it was found that as the diameter of TSs decreased, the probability of flow gap occurrence increased. A decrease in the mean diameter of the evaluated SSs in 3D MP-RAGE MRV was also found to be associated with an increased probability of flow gap occurrence in the corresponding TS. A statistically significant relationship was obtained between the asymmetry degree of evaluated TSs in 3D MP-RAGE MRV and the level of flow gaps evaluated in 2D TOF MRV. While the results obtained with this study demonstrate that contrast-enhanced T1-weighted 3D MP-RAGE MRV is over to 2D TOF MRV in CVS variation-thrombus differentiation, we believe that the combination of these two examinations will yield more reliable results. In addition to MRV, we anticipate that Doppler ultrasonography, which is a fast, easily accessible, and relatively inexpensive examination, will also be useful in distinguishing CVS variation-thrombus. Further comprehensive studies are needed in this regard.
