Çalışmamızda RA ve PsA'lı hastalarda düşme sıklığının belirlemesi, düşme risk faktörlerinin araştırılması ve düşmelerin hastalık aktivitesi ile ilişkisinin değerlendirmesi amaçlanmaktadır. RA tanılı 100, PsA tanılı 80 hasta değerlendirildi. Düşme öyküsü varlığı, komorbid hastalıklar, geçirilmiş operasyonlar, kullanılan ilaçlar kaydedildi. Hastalık aktivitesi; akut faz reaktanları, otoantikorlar, ŞES, HES, VAS, EGA, PGA, HAQ skoru ile değerlendirildi. RA için DAS28, CDAI, SDAI ve PsA için DAS28, PAŞİ, BASDAİ, ASDAS-CRP skorları hesaplandı. Düşme riski; TDYT, ZKYT, TDT, TYT, 5SOKT denge testleri ile değerlendirildi. Tüm bu parametrelerin düşme ile ilişkisi incelendi. Son 12 ay içerisinde RA hastalarının %33' ünde, PsA hastalarının %46,3' ünde düşme öyküsü mevcuttu. PsA' da osteoporoz ve daktilit varlığı ile düşme arasında anlamlı ilişki saptandı. RA ve PsA hastalarında görme bozukluğu ve antidepresan ilaç kullanımı düşme ile ilişkili saptandı. Düşme üzerine etkili olabilecek faktörler çoklu lojistik regresyon analizi ile değerlendirildiğinde; hastalık süresi ve HAQ skoru düşme ile ilişkiliydi. Hastalık aktivite skorları ile denge ve düşme testleri arasında yapılan korelasyon analizinde en güçlü korelasyon HAQ skoru ile saptandı. Denge testleri ile ESH, CRP, VAS, HAQ skoru, HES, DAS-28, CDAI, SDAI, EGA, BASDAİ ve ASDAS-CRP arasında istatistiksel olarak anlamlı ilişki bulundu. Düşme ile en güçlü korelasyon, HAQ skoru gibi hasta tarafından bildirilen sonuçlar için belirgindi. Hastalık aktivitesi artışı ve denge bozukluklarının düşme için risk faktörü olduğu görüldü. Sonuç olarak RA ve PsA hastalarının her muayene başvurusunda, düşme öyküsü ve düşme korkusu sorgulanmalıdır. Mümkünse her muayenede denge ve düşme testleri yapılarak düşme riskleri değerlendirilmelidir. Hastalık aktivitesinin yakın takip ile kontrol altına alınması, önlenebilir görme bozukluklarının tespiti için rutin tarama programları planlanması, erken dönemde kas güçlendirici fizik tedavi ve rehabilitasyon desteği sağlanması önemlidir. Düşmelerin kullanılan ilaçlar ve komorbit hastalıklar ile ilişkisini değerlendirilmesi için çalışmalara ihtiyaç vardır. In our study, we aimed to determine the incidence of falls in patients with RA and PSA, to investigate the risk factors of falling and to evaluate the relationship between falls and disease activity. 100 patients with RA, 80 patients with PsA were evaluated. The presence of a falling story, comorbid diseases, operation history and drugs used. Disease activity was evaluated with acute phase reactants, autoantibodies, TJS, SJS, VAS, EGA, PGA, HAQ score. For RA DAS28, CDAI, SDAI and for PSA DAS28, PASI, BASDAI, ASDAS-CRP scores were calculated. Fall risk were evaluated with balance tests such as TDYT, ZKYT, TDT, TYT, 5SOKT. The relationship between all these parameters and falls was examined. In the last 12 months, %33 of RA, %46,3 PsA patients had a falling history. In PSA, a significant relationship was found between the presence of osteoporosis and dactylitis falling. Visual impairment and antidepressant drug use in RA and PSA patients were associated with falling. When the factors that may be effective on falling are evaluated by multiple logistics regression analysis; the duration of the disease and the HAQ score were associated with falling. In the correlation analysis between disease activity scores and balance and fall tests, the most powerful correlation was determined by the HAQ score. A statistically significant correlation was found between balance tests and ESR, CRP, VAS, HAQ score, TJS, DAS28, CDAI, SDAI, EGA, BASDAI and ASDAS-CRP. The most powerful correlation with the fall was evident for the results reported by the patient, such as the HAQ score. Increased disease activity and balance disorders were a risk factor for falls. As a result, each examination of RA and PSA patients should be questioned in the application of falling and fear of falling. If possible, balance and fall tests should be evaluated on each examination. It is important to plan the disease activity with close follow -up, to plan routine screening programs for the detection of preventive visual disorders, and to provide muscle strengthening physical therapy and rehabilitation support in the early period.
