Giriş ve Amaç: Karın ağrısı acil servise en fazla başvuru sebepleri arasında yer almaktadır. Acil servis başvurularının %5-10'unu oluşturur. Acil servis başvurusunda karın ağrılı hastaların tanısı (cerrahi, dahili karın ağrıları) ve tanılara bağlı erken müdahale oldukça önemlidir. Neopterin hücresel immün sistemin aktivasyonu sonucu vücutta yükselen bir biyomarkerdır. Otoimmün hastalıklarda, viral, renal, kardiyak hastalıklarda ve farklı infeksiyon durumlarında vücutta yükselir. İnsan vücudunda serumdan ve idrardan ölçülür. Yüksekliği vücutta inflamasyonun varlığını gösterir. Bu nedenle karın ağrısı ile acil servise başvuran hastaların yapılan tetkiklerinde neopterin düzeylerin artıp artmadığı hastanın klinik ile korelasyonu ve acilden cerrahiye gidip gitmeyeceğine araştırmak amacıyla yapıldı. Karın ağrısı nedeniyle acil servise başvuran hastaların serum neopterin düzeylerinin hem tanıda hem de prognozun belirlenmesine yardım edebilecek biyolojik bir belirteç bulmayı amaçladık. Materyal ve Metot: Süleyman Demirel Üniversitesi Tıp Fakültesi Acil Servisi'ne karın ağrısından dolayı 1 Mart 2022 ile 1 Eylül 2022 tarihleri arasında başvuran 18 yaş ve üzeri 90 hasta ile 90 sağlıklıdan oluşan kontrol grubu alınıp araştırma yapılmıştır. Araştırma için sözel bilgilendirmeden sonra 90 hasta yazılı onamla venöz kan örneği vermiştir. Biyokimya tüpüne alınarak, örneklerin serumunun ayrıştırılmasının ardından ELİSA yöntemiyle neopterin seviyesi incelenmiştir. Bulgular: Çalışmaya 1 Mart-1 Eylül 2022 tarihleri arasında alınan 90 hastanın 48'i kadın, 42'si erkek; hastaların genel yaş ortalaması 55.48±22.61 olan çalışmamız prospektif olarak incelendi. Hastaların başvurusunda karın ağrısına eşlik eden en sık bulgu olarak bulantı, en az eşlik eden semptom ise ateş ve ishal olarak tespit edildi. Hastalarda en sık fizik muayene bulgusu hassasiyet bulundu. Karın ağrısına sahip hasta grubun sağlıklı gruptan ayrımının yapılmasında neopterin düzeyi istatistiksel olarak anlamlı bulundu. Acil servis de apandisit, barsak hastalıkları, safra kesesi hastalıkları, renal hastalıklar, non-katagorize karın ağrılı tanısı almış hastaların sağlıklı kontrol grubu ile kıyaslamalarına bakıldığında hastaların neopterin değerleri anlamlı bulundu. Çalışmaya dahil ettiğimiz diğer enflamatuar parametrelere göre neopterin değerinin daha sensitiv olduğu tespit ettik. Neopterinin cerrahi nedenli karın ağrılarını tespit etme konusun da dahili hasta gruba göre daha duyarlı ve güvenilir olduğunu söyleyebiliriz. Sonuç: Tek başına karın ağrılı hastalarda acilden cerrahiye gidip gitmeyeceğine karar vermek amacıyla neopterinin acil servislerde ayrıcı tanıda hücresel immün aktiviteye dayanarak istatistiksel olarak anlamlı bulduk. Bu konu ile alakalı daha sonra gerçekleştirilecek birçok çalışma ile neopterin değerinin standardizasyonu sağlanması gerekmektedir. Karın ağrılarında neopterin değerinin duyarlılığı ve özgüllüğü netleştirilmelidir. Bu duruma bağlı olarak, acil cerrahi ye yönlendirecek tanı sürecinde hedeflediğimiz biyokimyasal enflamatuar markerların yüksekliğinin tanısal ve prognostik etkisini ve tanı sürecinin hızlandırılmasının sağlanabileceğini umut ediyoruz. Introduction and Aim: Abdominal pain is one of the most common reasons for admission to the emergency department. It constitutes 5-10% of emergency department admissions. Diagnosis of patients with abdominal pain (surgery, internal abdominal pain) and early intervention depending on the diagnosis are very important in emergency service admissions. Neopterin is a biomarker that rises in the body as a result of the activation of the cellular immune system. It is elevated in the body in autoimmune diseases, viral, renal, cardiac diseases and different infections. It is measured in the human body from serum and urine. Its height indicates the presence of inflammation in the body. For this reason, it was carried out to investigate whether the neopterin levels increased in the examinations of patients who applied to the emergency department with abdominal pain, the correlation of the patient with the clinic and whether the patient would go to the emergency room or not. We aimed to find a biological marker that can help in the diagnosis and prognosis of serum neopterin levels of patients admitted to the emergency department with abdominal pain. Material and Method: A study was conducted with 90 patients aged 18 years and over and 90 healthy control groups who applied to the Suleyman Demirel University Medical Faculty Emergency Service between March 1, 2022 and September 1, 2022. After verbal information for the study, 90 patients gave venous blood samples with written consent. After separating the serum of the samples by taking them into a biochemistry tube, the neopterin level was examined by ELISA method. Results: Of the 90 patients included in the study between March 1 and September 1, 2022, 48 were female and 42 were male; Our study, whose mean age of the patients was 55.48±22.61, was evaluated prospectively. Nausea was the most common finding accompanying abdominal pain, and fever and diarrhea were the least accompanying symptoms. Tenderness was the most common physical examination finding in the patients. Neopterin level was found to be statistically significant in differentiating the patient group with abdominal pain from the healthy group. When the patients diagnosed with appendicitis, bowel diseases, gallbladder diseases, renal diseases and non-categorized abdominal pain in the emergency room were compared with the healthy control group, the neopterin values of the patients were found to be significant. We found that neopterin value was more sensitive than other inflammatory parameters included in the study. We can say that neopterin is more sensitive and reliable in detecting surgical-induced abdominal pain than the internal patient group. Conclusion: We found that neopterin was statistically significant in the differential diagnosis of emergency services, based on cellular immune activity, in order to decide whether to go to surgery from the emergency room in patients with abdominal pain alone. Standardization of neopterin value should be ensured with many studies to be carried out later on this subject. The sensitivity and specificity of the neopterin value in abdominal pain should be clarified. Depending on this situation, we hope that the diagnostic and prognostic effect of the high level of biochemical inflammatory markers that we target in the diagnosis process that will lead to emergency surgery and accelerate the diagnostic process.
