Amaç: Tıkanma sarılığında mortaliteye sebep olan asıl olay endotoksemi olup endotoksemi ile başlayan süreç sistemik etki yaratmaktadır. Kekik yağının literatürde antiinflamatuar ve antibakteriyel etkileri bildirilmiştir. Kekik yağının bu etkileri göz önünde bulundurulduğunda kekik yağının tıkanma ikterinde ortaya çıkan bakteriyel translokasyon ve sonrasında gelişebilecek hasarları önlemede kullanılabileceği düşünülmüş olup bu çalışmada amacı böyle bir etki olup olmadığını test etmektir. Gereç ve Yöntem: Bu çalışmada 250 – 300 gram ağırlığında, Wistar - Albino cinsi 28 adet sıçan, Sham (n:10), Koledok (n:11) ve Kekik (n:7) olacak şekilde 3 gruba ayrılmıştır. Kekik grubuna 6 gün boyunca oral olarak 50 mg/kg/gün Kekik Yağı verilmiştir. 6. günün sonunda alınan kan numunelerinden Total bilirubin (T.Bil), Alkalen Fosfataz (ALP), Gama Glutamil Transferaz (GGT), Alanin Aminotransferaz (ALT), Aspartat Aminotransferaz (AST) değerlerine bakılmıştır. Mezenter lenf nodu (MNL), dalak ve kan örnekleri alınarak mikrobiyolojik olarak uygun kültürlere ekilerek mikroorganizma üremeleri değerlendirilmiştir. Bulgular: AST, ALT,ALP, total bilirubin ve GGT değerleri gruplar arasında istatistiksel olarak anlamlı bir fark göstermektedir (p<0,05). Bonferroni düzeltmesi ile farkın hangi gruplar arasında olduğuna bakıldı. Koledok grubunun sham grubuna göre anlamlı biçimde daha yüksek olduğu tespit edildi. Kekik grubu ile koledok grubu arasında AST değerleri için anlamlı bir fark tespit edilmedi. Ancak medyan değerlerine bakıldığında kekik grubu AST (Med:630,4) değerinin koledok grubu AST değerine (Med:777,6) göre daha düşük olduğu tespit edildi. Kekik grubu ile koledok grubu arasında ALT değerleri için anlamlı bir fark tespit edilmedi. Ancak medyan değerlerine bakıldığında kekik grubu ALT (Med:200,6) değerinin koledok grubu ALT değerine (Med:286,5) göre daha düşük olduğu tespit edildi. Kekik grubu ile koledok grubu arasında ALP değerleri için anlamlı bir fark tespit edilmedi. Ancak medyan değerlerine bakıldığında kekik grubu ALP (Med:170,1) değerinin koledok grubu ALP değerine (Med:293,9) göre daha düşük olduğu tespit edildi. Kekik grubu ile koledok grubu arasında total bilirubin değerleri için anlamlı bir fark tespit edilmedi. Ancak medyan değerlerine bakıldığında kekik grubu total bilirubin (Med:2,1) değerinin koledok grubu total bilirubin değerine (Med:11,4) göre daha düşük olduğu tespit edildi. Kekik grubu ile koledok grubu arasında GGT değerleri için anlamlı bir fark tespit edilmedi. Ancak medyan değerlerine bakıldığında kekik grubu GGT (Med:0,4) değerinin koledok grubu GGT değerine (Med:18,3) göre daha düşük olduğu tespit edildi. Dalak doku kültürü değerlendirildiğinde sham grubunda 10 hayvanın 2'sinde,koledok grubunda 11 hayvanın 5'inde,kekik grubunda ise 7 hayvanın sadece 1'inde üreme olduğu gözlendi. Dalak doku kültürü sonuçlarına göre deney grupları arasında istatistiksel anlamlı bir fark saptanmadı (p:0,273). MLN doku kültürü değerlendirildiğinde sham grubunda 10 hayvanın 2'sinde, koledok grubunda 11 hayvanın 4'ünde, kekik grubunda ise 7 hayvanın 2'sinde üreme olduğu gözlendi. MLN doku kültürü sonuçlarına göre deney grupları arasında istatistiksel anlamlı bir fark saptanmadı (p:0,315). Kan kültürü değerlendirildiğinde sham grubunda 10 hayvanın 3'ünde,koledok grubunda 11 hayvanın 7'sinde,kekik grubunda ise 7 hayvanın sadece 2'sinde üreme olduğu gözlendi. Kan kültürü sonuçlarına göre deney grupları arasında istatistiksel anlamlı bir fark saptanmadı (p:0,199). Sonuç: Bu çalışmada deneysel tıkanma sarılığı oluşturulan sıçanlarda kolestaza bağlı ve karaciğer hasarına bağlı olarak kanda AST, ALT, ALP, GGT, Total Bilirubin değerleri anlamlı olarak arttığı gözlendi. Oral kekik yağı verilen grupta bu değerlerin azaldığı ancak istatiksel olarak anlamlı azalmadığı gözlendi. Bu çalışmada kekik yağı ile beslenen grupta dalak, MLN ve kanda tıkanma sarılığı oluşturulan gruba göre bakteriyel üremenin anlamlı olarak azalmadığı görüldü. Mesenteric lymph node (MLN), spleen, and blood samples were collected and microbiologically cultured to assess microbial growth. Findings: AST, ALT, ALP, total bilirubin, and GGT values show a statistically significant difference among the groups (p<0.05). Differences between groups were examined with Bonferroni correction. It was found that the Common bile duct group was significantly higher compared to the Sham group. There was no significant difference in AST values between the Thyme and Common bile duct groups. However, when median values were examined, it was found that the AST value of the Thyme group (Med:630.4) was lower compared to the AST value of the Common bile duct group (Med:777.6). There was no significant difference in ALT values between the Thyme and Common bile duct groups. According to the MLN tissue culture results, there was no statistically significant difference among the experimental groups (p: 0.315). When evaluating blood culture, it was observed that 3 out of 10 animals in the Sham group, 7 out of 11 animals in the Common bile duct group, and only 2 out of 7 animals in the Thyme group showed growth. According to the blood culture results, there was no statistically significant difference among the experimental groups (p: 0.199) Conclusion: In this study, it was observed that in rats with experimentally induced obstructive jaundice, AST, ALT, ALP, GGT, and Total Bilirubin levels significantly increased due to cholestasis and liver damage. In the group receiving oral thyme oil, these values decreased, although the decrease was not statistically significant. In the thyme oil-fed group, it was observed that bacterial growth did not significantly decrease in the spleen, MLN, and blood compared to the group with obstructive jaundice.
