Tip 2 diabetes mellitus; genetik, metabolik, sosyal, davranışsal ve kültürel faktörlerin etkileşimi sonucu gelişen komplike, mültifaktöriyel bir hastalıktır. T2DM başta gelişmiş ülkeler olmak üzere tüm dünyada prevalansı giderek artan bir sağlık sorunudur. Grelin, enerji dengesinin kontrolü, iştah kontrolü ve vücut ağırlığının düzenlenmesindeki rolü yoluyla T2DM'nin gelişimini etkileyebilir. Grelinin etkilerine büyüme hormonu sekretagog reseptörü (GHSR) aracılık eder. Bu vaka-kontrol kesitsel ilişkilendirme çalışmasına, Isparta, Türkiye popülasyonundan yetmiş beş T2DM hastası ve yirmi beş sağlıklı kontrol dahil edildi. Tüm denekler, grelindeki rs26311 (-1062C/G, promotor bölge), rs26312 (-994C/T, promotor bölge), rs27647 (-604G/A, promotor bölge), rs35683 (A/C, intronik bölge) ve rs3755777 (-1500C/G, promotor bölge) genetik varyantlar ve GHSR'deki rs509035 (g.7798C/T, intronik bölge) ve rs565105 (g.9130G/A,G/C,C/T, 3'UTR bölgesi) matriks yardımlı lazer desorpsiyon iyonizasyon-uçuş zamanı kütle spektrometresi (MALDI-TOF MS) kullanılarak genotiplendi. Genetik verilerin analizi SNPStats programı kullanılarak yapıldı. Vaka kontrol çalışmasında elde edilen verilere göre grelin SNP rs27647'nin ve rs35683'ün T2DM ile ilişkili olduğu bulundu (Dominant modelde P = 0.001, olasılık oranı (OR) 0.30,% 95 güven aralığı (0,12–0,77); overdominant modelde P = 0.006, olasılık oranı (OR) 0.25,% 95 güven aralığı ((0.09-0.66), sırasıyla). Yapılan haplotip analizimizde ise, bir haplotip T2DM için en yüksek risk oluşturmaktaydı (p<0.0001, olasılık oranı (OR) 38,% 95 güven aralığı (0,9 – 250,25). Bu veriler, küçük örneklem boyutu nedeniyle ön çalışma olarak kabul edilmesi gerekmesine rağmen, rs27647 ve rs35683 grelin gen varyantlarının T2DM'ye genetik yatkınlığa katkıda bulunabileceğini öne sürmektedir. Type 2 diabetes mellitus; it is a complex, multifactorial disease that develops as a result of the interaction of genetic, metabolic, social, behavioral and cultural factors. T2DM is a health problem whose prevalence is increasing all over the world, especially in developed countries. Ghrelin may affect the development of T2DM through its role in controlling energy balance, appetite control, and body weight regulation. The effects of ghrelin are mediated by the growth hormone secretagogue receptor (GHSR). Seventy-five T2DM patients and twenty-five healthy controls in the population of Isparta, Turkey were included in this case-control cross-sectional association study. All subjects received rs26311 (-1062C/G, promoter region), rs26312 (-994C/T, promoter region), rs27647 (-604G/A, promoter region), rs35683 (A/C, intronic region), and rs3755777 (- 1500C/G, promoter region) genetic variants and rs509035 (g.7798C/T, intronic region) and rs565105 (g.9130G/A,G/C,C/T, 3'UTR region) matrix-assisted laser desorption in GHSR genotyped using ionization-time-of-flight mass spectrometry (MALDI-TOF MS). Analysis of genetic data was performed using the SNPStats program. According to the data obtained in the case control study, ghrelin SNP rs27647 and rs35683 were found to be associated with T2DM (P = 0.001 in the dominant model, odds ratio (OR) 0.30, 95% confidence interval (0.12-0.77); overdominant P = 0.006, odds ratio (OR) 0.25, 95% confidence interval ((0.09-0.66), respectively). In our haplotype analysis, a haplotype posed the highest risk for T2DM (p <0.0001, odds ratio (OR) 38, 95% confidence interval (0.9-250.25). These data suggest that ghrelin gene variants rs27647 and rs35683 may contribute to genetic susceptibility to T2DM, although they should be considered a preliminary study due to the small sample size.
