Çalışmamızda germe egzersizi ile darbeli elektromanyetik alan (DEMA) yönteminin kas yaralanması tedavisindeki etkinliklerinin değerlendirilmesi amaçlanmıştır. Çalışmamızda 5 adet ön, 64 adet ana çalışmada olmak üzere toplam 69 adet Wistar Albino cinsi rat kullanılmıştır. Ön çalışma ideal darbeli elektromanyetik alan tedavi süresinin belirlenmesi amacıyla planlanmış ve ratlar 5 gruba (hasar, DEMA1 saat, DEMA2 saat, DEMA4 saat, DEMA6 saat) ayrılmıştır. Ana çalışmada ratlar 8 gruba (Kas Hasarı+Kontrol 7. ve 14. gün grupları, Kas Hasarı+DEMAideal 7. ve 14. gün grupları, Kas Hasarı+Egzersiz 7. ve 14. gün grupları, Kas Hasarı+DEMAideal+Egzersiz 7. ve 14. gün grupları) ayrılmıştır. İskelet kası hasarı 18g biyopsi iğnesi kullanılarak oluşturulmuştur. Germe egzersizleri 10 tekrar şeklinde (her tekrar 1 dakika germe ve 30 saniye dinlenme), DEMA tedavisi ise ön çalışma ile belirlenen süre ile uygulanmıştır. Ratlar, 7. gün ve 14. günlerde tedavilerin etkilerine bakılmak üzere sakrifiye edilmiştir. Deney sonunda alınan kas dokularında genetik, histopatolojik ve immünohistokimyasal değerlendirmeler yapılmıştır. Ön çalışma sonuçlarına göre ideal DEMA süresinin 6 saat olduğu belirlenmiştir. Ana çalışma sonuçlarına göre mononükleer hücre infiltrasyonu, kas dejenerasyonu, atrofi ve nekroz açısından değerlendirildiğinde 7. ve 14. gün sonunda hasar grubundaki değerlerin tüm gruplardan anlamlı düzeyde yüksek olduğu tespit edilmiştir (p<0,001). Piknotik çekirdek açısından incelendiğinde 7. ve 14. gün sonunda tüm gruplardaki değerlerin kontrol grubundan anlamlı düzeyde yüksek olduğu görülmüştür (p<0,001). 7. gün sonunda vasküler endotelyal büyüme faktörü (VEGF) değerlerinin gruplar arasında anlamlı değişiklik göstermediği; fibroblast büyüme faktörü (FGF) değerlerinin ise hasar grubunda hem kontrol grubuna hem de Hasar+Egzersiz grubuna göre yüksek olduğu belirlenmiştir (p<0,05). 14. gün sonunda VEGF değerlerinin hasar grubunda DEMA grubu hariç diğer gruplardan anlamlı düzeyde yüksek olduğu (p<0,05), FGF değerlerinin ise hasar grubunda tüm gruplara göre anlamlı düzeyde yüksek olduğu görülmüştür (p<0,001). Dönüştürücü büyüme faktörü β1'in (TGF-β1) ve endotelyal nitrik oksit sentaz (eNOS) değerlerinin 7. gün ve 14. gün ekspresyonlarının hasar grubunda diğer gruplara göre istatistiksel olarak anlamlı bir artış gösterdiği saptanmıştır (p<0.001). Sonuç olarak bu çalışmada, kasta inflamatuar süreci dengelemesi, kas gelişimine olumlu etki yapması, iyileşmeyi hızlandırması, TGF-β1 sinyalizasyonunu azaltarak fibrozis gelişimini ve inflamatuar nedenli eNOS aktivitesini engellemesi nedeniyle DEMA ve germe egzersizinin kas yaralanmaları tedavisinde etkin olabileceği gösterilmiştir. Özellikle fibrozis gelişiminin azaltılmasında germe egzersizi ve DEMA'nın birlikte uygulanması daha etkindir. In our study, we purposed to evaluate the efficacy of stretching exercise and pulsed electromagnetic field (PEMF) method in the treatment of muscle injury. In our study, a total of 69 Wistar Albino rats were used, 5 of which were in the preliminary study and 64 in the main study. The preliminary study was planned to determine the ideal pulsed electromagnetic field treatment time and the rats were divided into 5 groups (damage, PEMF1 hour, PEMF2 hours, PEMF4 hours, PEMF6 hours). In the main study, animals were divided into 8 groups (Muscle Injury+Control Day 7 and 14 Groups, Muscle Injury+PEMFideal Day 7 and 14 Groups, Muscle Injury+Exercise Day 7 and 14 Groups, Muscle Injury+PEMFideal+Exercise 7. and Day 14 Groups). Skeletal muscle injury was established using an 18g biopsy needle. Stretching exercises were applied as 10 repetitions (1 minute stretching and 30 seconds rest for each repetition), and PEMF treatment was applied for the time determined by the preliminary study. Experimental animals were sacrificed on the 7th and 14th days to see the effects of the treatments. At the end of the experiment, genetic, histopathological and immunohistochemical evaluations were made in the muscle tissues taken. According to the preliminary study results, it was determined that the ideal PEMF time was 6 hours. According to the results of the main study, when evaluated in terms of mononuclear cell infiltration, muscle degeneration, atrophy and necrosis, the values in the damage group were found to be significantly higher than all groups at the end of the 7th and 14th days (p<0,001). When examined in terms of pycnotic core, it was observed that the values in all groups were significantly higher than the control group at the end of the 7th and 14th days (p<0.001). At the end of the 7th day, vascular endothelial growth factor (VEGF) values did not differ significantly between the groups; fibroblast growth factor (FGF) values were found to be higher in the injury group compared to both the control group and the injury+exercise group (p<0.05). At the end of the 14th day, VEGF values were found to be significantly higher in the injury group than the other groups except the DEMA group (p<0.05), and FGF values were significantly higher in the injury group compared to all groups (p<0.001). It was determined that the expressions of transforming growth factor beta 1 (TGF-β1) and endothelial nitric oxide synthase (eNOS) values on the 7th and 14th days showed a statistically significant increase in the damage group compared to the other groups (p<0.001). In conclusion, in this study, it has been shown that PEMF and stretching exercise can be effective in the treatment of muscle injuries, as they balance the inflammatory process in the muscle, have a positive effect on muscle development, accelerate healing, prevent fibrosis development by reducing TGF-β1 signaling and inhibit inflammatory-induced eNOS activity. The combination of stretching exercise and PEMF is more effective, especially in reducing the development of fibrosis.
