Amaç: İn vivo olarak; 5-7 yaş grubundaki çocukların çürük süt molar dişlerinin sınıf II kavitelerinde, biri sonik sistem, diğeri el aletleri ile olmak üzere iki farklı teknikle uygulanan nanohibrit rezin materyallerin 18 aylık klinik başarılarının incelenmesi ve in vitro olarak; bu materyallerin mikrosızıntılarının karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: İn vivo olarak, kliniğimize başvuran, 5-7 yaşlarında, herhangi bir sağlık problemi olmayan 116 hastanın, arayüz çürüğü bulunan toplam 232 süt molar dişi, bulk fill (Sonic Fill® ) ve geleneksel (ReliaFil LC® ) olmak üzere 2 farklı kompozit rezin ile restore edildi. Bu restorasyonların 18 aylık klinik başarıları, USPHS (United States Public Health Service) kriterlerine göre incelendi. Elde edilen veriler, 2 oran 'z testinden' ile istatistiksel olarak değerlendirildi. İn vitro olarak, in vivo çalışmada kullanılan kompozit rezinler ile, 32 adet süt molar dişin arayüz kaviteleri restore edildi. Örnekler, 500 devirlik termal siklus uygulamasından sonra, % 0,5'lik bazik fuksin solüsyonunda 24 saat bekletildi ve mezial-distal yönde kesitler alındı. Stereomikroskop altında incelenen örneklerin mikrosızıntı skorları, MannWhitney U testi ile istatistiksel olarak değerlendirildi. Bulgular: Çalışmanın in vivo kısmında, her iki materyal ile yapılan restorasyonların retansiyon kriteri açısından 6., 12. ve 15., kenar renklenmesi kriteri açısından 12. ve 18., renk uyumu kriteri açısından ve yüzey yapısı kriteri açısından 12., 15. ve 18. aylarda görülen skorlar arasındaki farklılık, istatistiksel olarak anlamlı bulunmamıştır (p>0,05). Çalışmanın in vitro kısmında, her iki materyal ile yapılan restorasyonların gingival bölgelerindeki mikrosızıntı seviyelerinin daha fazla olduğu görülmüş olıp, materyaller arasında istatistiksel olarak anlamlı bir farklılık bulunmamıştır (p>0,05). Sonuç: Bulk fill ve geleneksel kompozit rezin restorasyonlarının 18 aylık klinik takibi ve in vitro olarak restore edilen dişlerin mikrosızıntılarının incelenmesi sonucunda, her iki materyalin de süt molar dişlerinin arayüz çürüklerinde başarı ile kullanılabileceği ve başarı kriterleri açısından istatistiksel olarak bir farklılık göstermedikleri görülmüştür. Ancak, çocuk hastada uygulama kolaylığı göz önüne iv alındığında, bulk fill kompozit rezin olan Sonic Fill® 'in bu açıdan avantajlı olduğu görülmesine karşın, bu materyal ile uzun takipli klinik çalışmalarının daha çok yapılmasına ihtiyaç vardır. Aim: In the in vivo part of the study; investigation of 18-month clinical success of nanohybrid resin materials applied with two different techniques, one sonic system and the other hand instrument, in class II cavities of carious primary molar teeth of children aged 5-7 years, and in vitro part; It is aimed to compare the microleakage of these materials. Materials and Methods: In the in vivo part of the study, a total of 232 primary molar teeth with interface caries of 116 patients, aged 5-7 years, without any health problems, who applied to our clinic, were restored with 2 different composite resins, bulk fill (Sonic Fill® ) and conventional (ReliaFil LC® ). The 18-month clinical success of these restorations was evaluated according to USPHS (United States Public Health Service) criteria. The obtained data were statistically evaluated with 2 ratio 'z-tests'. In the in vitro part of the study, the interface cavities of 32 primary molars were restored with the composite resins used in the in vivo study. After 500 cycles of thermal cycle application, the samples were kept in 0,5 % basic fuchsin solution for 24 hours and sections were taken in the mesial-distal direction. Microleakage scores of the samples examined under the stereomicroscope were statistically evaluated with the Mann-Whitney U test. Results: In the in vivo part of the study, restorations made with both materials were 6th, 12th, and 15th in terms of retention criteria, 12th and 18th in terms of marginal discoloration, 12th, 15th, and 18th in terms of color matching criteria, and surface structure criteria. The difference between the scores observed at the 12th, 15th and 18th months was not statistically significant (p>0,05). In the in vitro part of the study, it was observed that the microleakage levels in the gingival regions of the restorations made with both materials were higher, and there was no statistically significant difference between the materials (p>0,05). Conclusion: In a conclusion of 18-month clinical follow-up of bulk fill and conventional composite resin restorations and evaluations of microleakage of teeth vi restored in vitro, it was seen that both materials could be used successfully in interface caries of primary molar teeth and did not show any statistical difference in terms of success criteria. However, considering the ease of application in pediatric patients, Sonic Fill® , a bulk fill composite resin, seems to be advantageous in this respect, but more clinical studies with long follow-ups are needed with this material.
