Kronik boyun ağrısı ve ilişkili postür problemleri respiratuar ve fonatuar mekanizmaları etkilemektedir. Bu problemler kişilerin ses kontrolü ve rezonans üzerinde etkili olan farinks ve larinks çevresi yumuşak doku ve kas yapılarında disfonksiyonlara neden olmakta ve ses kalitesini etkilemektedir. Çalışmamızın amacı servikal stabilizasyon egzersizlerinin fonatuar mekanizma ve ses kalitesi üzerine etkilerini göstermektir. Çalışmamıza spesifik olmayan kronik boyun ağrılı 38 hasta dahil edilmiştir. Katılımcıların demografik bilgileri ve ağrı, ses hijyeni, egzersiz alışkanlığı gibi klinik durumları kaydedilmiştir. Katılımcılara uygulanan klinik test ve değerlendirmeler sırasıyla; akustik ses analizi, aerodinamik değerlendirme, ses handikap endeksi, ağrı şiddetleri, hasta sağlığı anketi-kısa formu, McGill ağrı anketi, Boyun Ağrı ve Yetersizlik Skalası ve Fremantle Boyun Farkındalık Anketi'ni içermektedir. Ayrıca katılımcıların servikal bölge kas enduransı, boyun eklem hareket açıları ve baş postürleri değerlendirilmiştir. Bu değerlendirmeler tedavi öncesi ve sonrası olmak üzere iki defa uygulanmıştır. Kornik boyun ağrılı hastalar randomize olarak; servikal stabilizasyon egzersiz grubu (n:20) ve geleneksel egzersiz grubu (n:18) olmak üzere ikiye ayrılmıştır ve hastalara ilk üç hafta haftada beş, ikinci üç hafta haftada 3 seans olmak üzere, altı hafta boyunca toplam 24 seans egzersiz programı uygulanmıştır. Servikal stabilizasyon egzersizleri ve geleneksel egzersizler hastaların eklem hareket açıklığını arttırmış, servikal postürünü düzeltmiş ve ağrılarını azaltmıştır (p<0,005). Servikal stabilizasyon egzersizleri ağrı, fonksiyonellik ve endurans parametreleri açısından geleneksel egzersizlerden etkili bulunmuştur (p<0,05). Egzersizler hastaların akustik değerlendirmelerini etkilemese de (p>0,05) aerodinamik parametrelerinde (MFZ ve s/z oranları) ilerleme sağlamıştır (p<0,05). Ayrıca hastalara uygulanan egzersizler boyun ağrısı ile ilişkili dizabilite ve depresyon durumunu iyileştirmiştir (p<0,05). Sonuç olarak bu çalışmada spesifik olmayan kronik boyun ağrılı hastalara uygulanan servilkal stabilizasyon egzersizleri ve geleneksel egzersizlerin hastaların servikal postural düzgünlüğünü ve boyun ağrısına bağlı kliniğini iyileştirmiştir. Böylece sesin aerodinamik parametrelerinde gelişmeler sağlanmıştır. Ses bozukluğu yaşayan kişilerin tedavisinde stabilizasyon egzersizlerinin tedavinin parçası olması gerektiği düşünülmektedir. Chronic neck pain and related postural problems affect respiratory and phonatory mechanisms. These problems cause dysfunctions in the soft tissue and muscle structures around the pharynx and larynx, which are effective on voice control and resonance, and affect the voice quality. The aim of our study is to show the effects of cervical stabilization exercises on phonatory mechanisms and voice quality. 38 patients were included in our study. Participants' demographic information and clinical status (pain, voice hygiene, exercise habits) were recorded. The clinical tests and evaluations were applied to the participants, respectively; acoustic analysis, aerodynamic assessment, voice handicap index, pain severity, patient health questionnaire-short form, McGill pain questionnaire, Neck Pain and Disability Scale, and Fremantle Neck Awareness Questionnaire. In addition, cervical region muscle endurance, neck goniometric joint movement measurements, and head posture were evaluated. These evaluations were applied twice, before and after the treatment. Patients with cornic neck pain were randomized; it was divided into two groups as the Cervical Stabilization Exercise Group (n:20) and the Traditional Exercise Group(n:18), and the patients were given an exercise program for a total of six weeks, five times a week for the first three weeks and three times a week for the second three weeks. Cervical stabilization and traditional exercises increased the patient's range of motion, improved their cervical posture, and reduced their pain (p<0.05). Cervical stabilization exercises were found to be more effective than traditional exercises in terms of pain, functionality, and endurance (p<0.05). Although the exercises did not affect the acoustic evaluations of the patients (p>0.05), they improved aerodynamic parameters (MFZ and s/z ratios) (p<0.05). In addition, the exercises applied to the patients also improved the disability and depression associated with neck pain (p<0.05). As a result, in this study, cervical stabilization and traditional exercises applied to patients with non-specific chronic neck pain improved the cervical postural straightness of the patients and the clinic related to neck pain, thus improving the aerodynamic parameters of the voice. It is suggested that stabilization exercises should be a part of the treatment in the treatment of people with voice disorders.
