Bu araştırmanın amacı, kronik böbrek yetmezliği (KBY) hastalığında uygulanan hemodiyaliz (HD) ve periton diyalizi (PD) tedavi yöntemlerinin geri ödeme kurumu perspektifinden maliyet etkililiğini ve bütçe etkisini analiz etmektir. Araştırmanın evrenini 2021 yılında Antalya il merkezindeki Akdeniz Üniversitesi Hastanesi ve Antalya Özel RDS Diyaliz Merkezinde HD ve PD tedavisi alan hastalar oluşturmaktadır. Araştırmada evrenden örneklem çekilmiş HD ve PD tedavisi alan 296 hastaya yaşam kalitesi ölçeği uygulanmıştır. Diyaliz tedavi seçeneklerinin maliyeti ve etkililiği Markov modeli ile tespit edilmiştir. Araştırmada kullanılan maliyet verileri hasta faturalarından, etkililik değerleri ise hastalara uygulanan EuroQol-5D-5L ölçeği ile tespit edilmiştir. Modelde hastaların sağlık durumları arasındaki geçişleri için döngü uzunluğu 1 yıl, zaman dilimi ise 30 yıl olarak belirlenmiştir. Araştırmada diyaliz tedavi seçeneklerinin toplam maliyeti ve toplam kaliteye ayarlı yaşam yılı (QALY) hesaplanmış ve hesaplanan sonuçlara %6 indirgeme yapılmıştır. Sonuçlar ilave maliyet etkililik oranı (İMEO) olarak sunulmuştur. Ayrıca araştırmada ikinci sonuç ölçütü olarak ilave net parasal fayda (Net Monetary Benefit-NMB) hesaplanmıştır. Maliyet-etkililik analizinde tek yönlü ve olasılıksal duyarlılık analizi yapılmıştır. Olasılıksal duyarlılık analizinde maliyetler gamma, etkililik ise beta dağılımı kullanılmıştır. Araştırmada klinik etkililik oranı HD ve PD sırasıyla 0,70 ve 0,80 olarak tespit edilmiştir. Maliyet ise HD ve PD sırasıyla 133.058-TL ve 211.017-TL olarak saptanmıştır. Markov kohort modelinde geçiş olasılıkları esas alınarak hesaplanan yaşam yılları HD ve PD sırasıyla 9,61 ve 9,80 olarak QALY değerleri ise 7,13, PD ve 7,84 olarak tespit edilmiştir. Yapılan temel durum maliyet etkililik analizinde, PD tedavisi, HD tedavisine göre 0,71 QALY ilave etkililik ve 638.260-TL ilave maliyet getirmektedir. Eşik değer (WTP) 577.938-TL hesaplanmıştır. Araştırma sonucunda İMEO tutarı eşik değerin üzerinde çıkmıştır (İMEO>WTP=638.260>577.938). Bu nedenle PD tedavisinin HD tedavisine göre maliyet etkili bir strateji olmadığı tespit edilmiştir. Araştırmada ikinci sonuç ölçütü olarak NMB değerleri hesaplanmıştır. NMB değeri HD ve PD tedavileri için sırasıyla 2.537.075-TL ve 2.494.133-TL olarak bulunmuştur. Bu sonuca göre göre en yüksek NBM değerine sahip seçeneğin HD tedavisi olmasından dolayı PD stratejisi HD'ye karşı maliyet etkili bulunmamıştır. Yapılan tek yönlü duyarlılık analizinde sonuçların tüm parametre değişimlerine karşı duyarlı olduğu bulunmuştur. Olasılıklı duyarlılık analizinde ise 577.938/QALY'lik bir eşik değerde monte carlo simülasyonların %56,3 HD ve %43,7 PD daha maliyet etkili bir strateji olarak tespit edilmiştir. Bütçe etki analizinde 2022 yılındaki toplam sağlık harcamalarının %2,2'sinin HD ve PD tedavileri için yapıldığı tespit edilmiştir. Sonuç olarak HD ve PD tedavisi oldukça maliyeti yüksek sağlık teknolojileridir. Bu nedenle diyaliz tedavilerinin ilaç maliyetlerinin düşürülmesine yönelik sağlık politikalarının geliştirilmesi önerilmektedir. The purpose of this research is to analyse the cost-effectiveness and budgetary implications of hemodialysis (HD) and peritoneal dialysis (PD) as treatment options for chronic kidney failure (CKF) from the viewpoint of the reimbursement institution. The population of the research consists of patients who received HD and PD treatment at Akdeniz University Hospital and Antalya Private RDS Dialysis Centre in 2021. In this study, 296 individuals who got treatment for HD and PD were selected as a sample from the entire population and a quality-of-life scale was applied to them. The cost and effectiveness of dialysis treatment options were determined by the Markov model. The cost data utilized in the research were derived from patients' invoices, while the effectiveness metrics were assessed using the EuroQol-5D-5L scale administered to the study participants. In the model, the cycle length for the transition of patients between health status is 1 year, and the time frame is 30 years. In the study, the total cost of dialysis treatment options and the total quality-adjusted life years (QALY) were calculated, and a 6% reduction was made to the calculated results. The study's findings are portrayed through the presentation of the additional cost-effectiveness ratio (IMEO), and as a supplementary outcome measure, the Additional Net Monetary Benefit (NMB) was calculated. In the cost-effectiveness analysis, one-way and probabilistic sensitivity analysis was performed. In the probabilistic sensitivity analysis, the gamma distribution of costs and the beta distribution of effectiveness were used. In this study, the clinical efficacy rates for HD and PD were determined as 0.70 and 0.80, respectively. The costs were determined as 133,058-TL and 211.017-TL, respectively, for HD and PD. In the Markov cohort model, life years HD and PD calculated based on transition probabilities were determined as 9.61 and 9.80, respectively, and QALY values were determined as 7.13 and 7.84. In the baseline cost-effectiveness analysis, PD treatment brings 0.71 QALY of additional effectiveness and an additional cost of 638.260-TL compared to HD treatment. The calculated threshold value, representing the willingness-to-pay (WTP), amounted to 577.938-TL. The research results showed that IMEO amount exceeded the threshold value (İMEO>WTP=638.260>577.938). Hence, it has been concluded that PD treatment is not deemed a cost-effective strategy when compared to HD treatment. Additionally, NMB values were computed as the secondary outcome measure in the study. Based on the findings, the NMB value was determined to be 2.537.075-TL for HD treatment and 2.494.133-TL for PD treatment. Consequently, it is evident that the HD treatment option exhibits a higher NMB value, indicating its cost-effectiveness superiority over PD treatment. In the one-way sensitivity analysis, the results were found to be sensitive to all parameter changes. In the probabilistic sensitivity analysis, 56.3% HD and 43.7% PD of Monte Carlo simulations were determined as a more cost-effective strategy at a threshold value of 577.938/QALY. The budget impact analysis revealed that HD and PD treatments accounted for 2.2% of the total health expenditures in 2022. Consequently, these treatments were identified as being relatively costly healthcare technologies.
